A
PUBLIC HtALTH SERVKIS
SAN JOAQUIN COUNTY z:
ENVIRONMENTAL HEALTH DIVISION " ` .;
Karen Furst, M.D., M.P.H., Health Officer
304 East Weber Avenue, Third Floor • Stockton, CA 952C. 0FO
FILE
R
209/468-3420
DOUGLAS DRIVER MAY 181999
FONZIA CORPORATION
1001 TAHOE BLVD .
INCLINE VILLAGE, NV 89451
Re: Fonzia Corp. (formerly Fortiber), 501 Acacia St., Tracy CA
San Joaquin County Public Health Services Environmental Health Division (PHS/EHD)
has reviewed the Work Plan for Soil and Water Investigation dated April 26. 1999
prepared by Geological Technics, Inc. The site history documentation indicates that
hazardous waste clean up activities took place in 1995 at the site. Chemicals other than
petroleum constituents and motor oil were identified as wastes at the site in 1995. The
clean up activities were not overseen by PHS/EHD. The potential for soil and water
contamination from unknown hazardous wastes previously generated at this facility,
including but not limited to ink wastes, glue wastes and heavy metals, exists at this site.
Additionally, Gail Wigget of the Central Valley Regional Water Quality Control Board
(CVRWQCB) has commented, apparently verbally, on the above referenced Work Plan
for this site and made recommendations for further testing of the soil and groundwater.
Due to the potential for non-petroleum and/or other unknown hazardous v,-aste
contamination of soil and water at this site, the CVRWQCB will be the lead agency. Any
"No Further Action" letter for this site as requested in your May 4, 1999 letter (FAX) to
PHS/EHD, must be prepared by the CVRWQCB.
Copies of CVRWQCB approved Work Plan(s) and any necessary boring permits should
be submitted to PHS/EHD. Notify PHS/EHD 48 hrs. in advance of any scheduled work
at this site. Please contact Carol Oz, Senior REHS if you have any questions about this
letter.
Donna Heran, RENS, Director
Environmental Health Division
Carol Oz, Senior REHS MT- re agorio, RE
Site Mitigation Unit IV Supervisor
cc: Ray Kablanow-Geologic Technics
Gail Wigget-CVRWQCB
A Division of San?oaquin Count• Health Care Ser.-:ces
