Louise Avenue <br /> Lathrop, CA i <br /> �z <br /> The Problem Assessment Report dated June 3, 1988,and the final Site Investigation Recommendations dated <br /> August 8, 1988, have been reviewed. f <br /> Pursuant to Section 2652(d) of the Underground Storage Tank Regulations (California Cody of Regulations, <br /> Title 23, Subchapter 16) Quarterly Monitoring Reports should be submitted to this office. The monitoring <br /> report shall be submitted to Public Health Services,Environmental Health Division (PHS/FJ D)'and Central <br /> Valley Regional Water Quality Control Board (CVRWQCB) until the investigation and cleanup of the site is <br /> deemed adequate. The report is to show the following: <br /> I� <br /> -Status of investigation and cleanup activities including the results of all investigations completed <br /> to date; !3 I <br /> -proposed continuing of next phase of investigation; <br /> -monitoring well data including groundwater elevation pH, conductivity and gradient; and 4 <br /> analytical results of groundwater or soil sampling.. _i I <br /> It must be shown that one of the existing monitoring wells is located in the verified downgradient from the <br /> source of contamination.
