In addition, the Health District has not received a <br /> report for the assessment and remediation work which has been performed <br /> to date. Submit the monitoring well results to this office within 15 <br /> days of the date of this letter. Submit a complete report of all site <br /> assessment and/or remediation activities (including soil excavation, <br /> system repairs, etc...) performed to date by June 15, 1988. Issuance of <br /> a permit to operate this facility is currently being delayed pending <br /> submission and evaluation of the requested information and demonstration <br /> of complaince with all monitoring and reportiing requirements of <br /> California Health & Safety Code Sections 25280 -- 25299.6 and the <br /> California Code of Regulations, Title 23, Chapter 3, Subchapter 16. <br /> Provide the report within fifteen days of reciept of this letter. If the <br /> report is not received within fifteen days the Health District will <br /> schedule an office hearing to discuss this matter. <br /> Pursuant to the San Joaquin Local Health District fee schedule Resolution <br /> No. 85-58, a $35.00 consultation fee shall be submitted along with the <br />'`. completed general Environmental Health Application form for staff review <br /> of the above referenced environmental assessment report. <br /> Each additional hour over the initial one hour of time spent in review <br /> activity will be billed at the rate of $35.00 per hour.
