;Ih
fi
s. rt
LEjo
Valem
Before Mr. Davis;,
will be allowed to load at the Stockton Terminal
°'again, we will need to have responses from Bartlett Petroleum on the
f 4,.following items:
i
Driver Training:
:The driver was unaware of retain in the truck and entered the wrong
!,presets for loading.
'The driver was not at; the meter which is a standard operating procedure of
:this facility, ( written policy included) . He was on the opposite side of
,the meter and loading, arm, talking with a driver in another lane . If he
had been at the loading side of the truck and at the meter, he would have
'been able to stop thei:joverflow sooner.
Mr, Davis will have to schedule a loading appointment, to be evaluated on '
Ehis knowledge of our .loading equipment and procedures prior to having full
access to loading privileges at this terminal again.
Ei
'}Bartlett Equipment: ih
-We would like to have! third party documentation that the high level
sensors in this semi have been checked since the spill and are
:operational . We would, iappreciate any other preventative maintenance
documentation you can', provide for the trucks that load at this terminal .
i$Bartlett Investigation:
:We would like to know:; that Bartlett also has investigated this incident
,'and will work with us' to avoid a repeat .
Valero LP is committed to maintaining the highest safety and environmental
'standards at it' s facilities . We feel we have fairly investigated ated this
E'incident, and believe:, it was caused by a combination of driver error and
equipment failure ( truck high level sensors) .
EE I do appreciatethe time and honesty you have given our concerns on
;this issue so far, Mr. Gomez, and I will be glad to assist with putting
4this incident behind us and moving on.
Mr. Davis will remain unable to load at this facility until we have
,received a written response to these requests that is acceptable.
i
f '
.Sincerely,
Shawn Mendoza
.Area Manager / West Region
Valero LP
I
CC: Mr. Rich Brandes %HSE Director - West Region
s
r I
