4 <br /> ValeroLrap <br /> Western Region 90 San Pablo Avenue <br /> HSE Office Crockett, CA 94525 <br /> 510-787-7294 <br /> 510-787-1205 fax <br /> INCIDENT INVESTIGATION REPORT <br /> No. 45952 <br /> Date July 18, 2006 <br /> From: Richard Brandes <br /> To: Jerry Stauffer, Mike Burgett <br /> Date/Time of Incident: July 17, 2006 8:00 pm <br /> Investigation Began: July 18, 2006 <br /> Location of Incident: Stockton Terminal <br /> Description of Incident <br /> Bartlett Petroleum driver Brian Davis was loading clear diesel into the third <br /> compartment of his truck when the operator on duty observed overflow <br /> from the truck dome. The operator stopped the flow of product to the truck <br /> with the emergency shutoff switch and alerted the driver. The rack was <br /> evacuated. Approximately 100-200 gallons of diesel were discharged into <br /> the containment area and collection sump. This incident is classified as a <br /> non-reportable discharge. However, the NRC and local agencies were <br /> provided courtesy notifications. <br /> Results of Investigation <br /> The Bartlett truck involved had retained product in the tank that overflowed, <br /> probably 800-900 gallons. Bartlett has confirmed the truck contained retain. <br /> The driver pre-set the meter to load more product than the tank would hold. <br /> It also appears that the truck overfill sensor did not function properly. A <br /> test of the terminal overfill sensing equipment and shut-offs indicates <br /> terminal equipment operated properly. <br /> Direct and Contributing Causes of the Incident <br /> • Actions of People: driver error— the driver was unaware of retain in the <br /> truck and entered the wrong presets for loading. <br /> • Equipment: the truck overfill sensors did not operate properly, which <br /> resulted in the overfill. <br /> Recommendations to Prevent Recurrence <br /> • The incident needs to be investigated by the carrier. The Bartlett <br /> training program needs to be addressed. In addition, it appears the <br /> carrier's sensor testing and truck maintenance program needs to be <br /> 45962 incident investigation report final 711912006 <br /> Page 1 of 2 <br />
