Environmental Health Department <br /> SAN,JOAQUIN <br /> —COUNTY - <br /> Complaint Record Daily Time and Activity Summary from 3/1/2015 to <br /> 6/21/2034 for CO0045285 located at <br /> 3 S PACIFIC AVE <br /> Activity Date <br /> Recorded by <br /> 12/15/2017 Received notification that at 5 AM today there was a container fire containing PE 2546 RIVERA <br /> 294 min chemicals that went down the storm drain at Lodi Unified. SC 544 <br /> 8:13 AM Called Mitch Slater. The fire occurred in the custodial storage container <br /> at Lodi High School, at the back of the school near the soccer field. A safety <br /> officer is keeping kids away from the area. A plumber is investigating the extent of <br /> the storm drain contamination and will use plugs to keep it from travelling, if <br /> needed. The container held cleaners, waxes, disinfectants, and miscellaneous <br /> other supplies. Ramos Environmental has been contacted to conduct clean up <br /> and will arrive around 10 or 11 AM. Jeff Culverson is the HAZWOPER trained <br /> person on scene. <br /> 9:49 AM Arrived on site. Met with Mitch Slater and Jill Davis, Risk Management <br /> Analyst for Lodi Unified. Liquid from fire clean up was observed on the ground <br /> amongst the burned waste. Some liquid spread to the soccer field behind the <br /> container. A small amount of liquid entered the storm drain just north of the <br /> container, but did not appear to spread to adjacent storm drains. Particles from <br /> the fire were observed on the soccer field behind the container. It was unclear <br /> whether the particles got there from wind or water, but no wind was noted while on <br /> site. Some water run off from the fire response was observed on the ground <br /> behind the container, in the soccer field area. The full extent of water run off <br /> contamination could not be determined. <br /> 10:15 AM Joe Ormonde with Ramos Environmental arrived. Ramos <br /> Environmental will handle the clean up of the waste, container, and storm drain, <br /> with assistance from R& B Environmental. Samples are being planned for the <br /> possible contamination of the soccer field. <br /> 10:30 AM Called the City of Lodi reporting system to notify them of the <br /> contamination in the storm drain system. Mitch Slater contacted a third party <br /> consultant to locate an ELAP certified lab for analytical of the soil samples. They <br /> are anticipating toxicity testing, including 96-hour bioassay. <br /> 10:43 AM Andrew Likewood with the City of Lodi (209-769-0053)called to get <br /> more information about the situation at the site. He called Mitch Slater and <br /> discussed the clean up with him. <br /> 11:15 AM 55 gallon drums were delivered to the site for containment of the fire <br /> debris as hazardous waste solids. All solids will be packaged and managed as <br /> hazardous waste. <br /> Issued a Notice to Abate for the spill. Copies were provided to Eric Wise and <br /> Mitch Slater with Lodi Unified. Ramos is planning to wait to do the storm drain <br /> cleaning until school gets out later this afternoon. Lodi Unified will provide sample <br /> jars for soil sampling later this afternoon, also. <br /> Safety data sheets were provided by Jill Davis for all of the chemicals that are <br /> stored in the container, which included dry deodorants, chewing gum&candle <br /> wax remover, foam hand & body shampoo, quat based disenfectant, dust cloth <br /> treatment, glass & multi-surface cleaner, surface creme cleanser, bathroom <br /> cleaner&scale remover, neutral floor cleaner, waterbased floor finish, <br /> non-ammoniated glass& multi-purpose cleaner, floor stripper, general purpose <br /> cleaner with hydrogen peroxide, Clorox disinfecting spray, and cleaning agent. Jill <br /> Davis and Joe Ormonde both stated they took pH samples of the liquid <br /> accumulated around the fire debris and found approximately pH 11. Left the site <br /> Page 2 of 3 <br />
