P Complaint Record Daily Time and Activity Summary from 5/11/2014 to <br /> 9�/FORS` 5/11/2222 for C00040255 located at <br /> 161 TRANSPORTATION <br /> Activity Date Recorded by <br /> 08/21/2015 17:42: received a page from the general hospital. Chief Tualla with French Camp PE 2546 MANZO <br /> Fire department is requiring assistance(209-401-3499).Called Chief Tualla and SC 974 <br /> left him a voice message. Received a call back from Chief Tualla, who is reporting <br /> that there is a hazardous material leak in the county right of way, some liquid had <br /> leaked out from the end of the box trailer parked on Transportation drive, they <br /> have not approached or opened the trailer, since a malicious intent is suspected. <br /> In addition, someone poured some sanitizer inside bathroom drains at the nearby <br /> business located at 161 Transportation Drive. The dumping resulted in a <br /> bleach-like fume that prompted the building evacuation. The strong odor was <br /> noted at 5 pm. It is my understanding that the liquid was dumped into the onsite <br /> toilets and sinks, no spilled liquid was noted on the bathroom floor; several people <br /> reported respiratory and breathing issues. Empty jugs of commercial bleach were <br /> found onsite. Called Kasey F. (Prog. Coordinator)and the haz. Mat. duty officer; <br /> relayed all the information. 18:45, arrived onsite. "Eco-San"from Ecolab was <br /> reported to have been disposed of into the onsite bathroom drains. The police <br /> (case number 15-201213, Cha Her deputy Sheriff), paramedics, county Haz. Mat <br /> and fire department are onsite. The facility is still evacuated, all employees are <br /> congregated in the yard area. Police were talking to the driver that stepped <br /> forward saying that he had disposed of the bleach into the bathroom area. Update <br /> to Kasey Foley. Spoke with the evacuated employees, indicated that they should <br /> seek medical attention if they have any symptoms or if they were to develop <br /> symptoms. The fire department reopened the building and let employees in once <br /> the area was ventilated. Haz mat(EHD)team onsite took the pH of the material <br /> stain (1x1 foot diameter) located on the ground next to the back of the box truck, <br /> pH was at B. The bleach solution that was dumped (per the located empty <br /> containers)was at 8.1% bleach and it would likely have pH over 12.5. The driver <br /> (Mr. Mike Delp, 209-614-6231, truck#9310, trailer#21754. <br /> roadservicesdepartment@marten.com;715-926-4216), employed by Marten <br /> Transpiration, indicated that he had some damaged containers and he received a <br /> directive from his corporate to dispose("get rid") of them. Mr. Delp indicated that <br /> he sold some of the containers(the total count of sold containers was not verified) <br /> and those he couldn't sell he disposed into the onsite bathroom drains. He showed <br /> me a directive he received from Marten dispatch, which stated to dispose or to <br /> recycle the material appropriately and to not dispose to drains. I indicated that it <br /> says to"not dispose to drains", to which he indicated he thought they mean storm <br /> drains. I asked if he would email me this communication, to which he indicated <br /> that he would; I provided my email address(no above communication was <br /> received).Three 1-gallon empty containers of"Eco-San" and one 2.5-gallon <br /> container of"Sanitizing Wash N Walk"were discovered inside the garbage <br /> container located at 161 Transportation. It was my understanding that the two <br /> liquids commingled at the time of the disposal. According to the container label, <br /> these containers have to be triple rinsed prior to their disposal or recycling. These <br /> containers were removed from the onsite garbage and containerized inside the <br /> box trailer owned by Marten Transportation, pending disposal. Prepared a notice <br /> to abate for illegal disposal, provided a copy to Mr. Mike Delp. Called Nick <br /> Sergerstrom(weekend supervisor at Marten, 800-395-3000(ext 2431), discussed <br /> the incident. Documented the spill. Revised the NTA(to add more information, <br /> since the driver wanted to leave the site asap I wasn't able to include all the <br /> details). Emailed a copy of the NTA to the Nick Sergerstrom, <br /> (sergerstrom.nas@marten.com; Nicholas.Segerstrom@marten.com), the <br /> weekend supervisor. <br /> Page 2 of 4 <br />
