O p4UIN C <br /> San Joaquin County <br /> Q: <br /> Environmental Health Department <br /> P Complaint Record Daily Time and Activity Summary from 5/1,V2014 to <br /> '`rFaR� 5/11/2222 for C00040255 located at <br /> 161 TRANSPORTATION <br /> Activity Date Recorded by <br /> 08/21/2015 Hazardous materials response to Delta Truck Center; arrived on scene and PE 2546 LOPEZ <br /> received briefing from IC; questioned RP; RP said he disposed of 4 containers of SC 904 <br /> disinfectant in the bathroom by diluting them with water; RP said there were was a <br /> case of material still in his trailer; French Camp Fire opened the back of the trailer <br /> and there was one case(4 gallons) of 8.4 % of sodium hypochlorite material <br /> present; Elena wrote the NTA; Fire located the empty containers, I took photos. <br /> 08/22/2015 10:40: Received a phone call from Nick Sergerstrom with Marten, discussed the PE 2546 MANZO <br /> incident and the plan for disposal of the dumped containers, Marten is looking into SC 944 <br /> hiring a hazardous waste contractor, they will call me back later today to provide <br /> an update. 12:57: received a phone call from Nick Sergerstrom (with Marten), he <br /> indicated that they will be retaining Haz. Mat 1 (800-229-5252), a licensed HW <br /> hauler that would deal with containers. Called Haz. Mat 1, spoke with David Peck <br /> regarding the incident. He indicated that Morgan Environmental will be sub- <br /> contracted to dispose of the empty containers, David also indicated that since it's <br /> not practical to triple rinse these containers onsite, they will haul away these <br /> containers on hazardous waste manifest and will likely triple rinse in Hayward at <br /> Morgan environmental location. David indicated that"Sanitizing Wash N Walk" <br /> container is likely ammonia based, it appears that if the"Sanitizing Wash N Walk" <br /> and "Eco San" were mixed together it would have created chlorine gas. <br /> Researched "Sanitizing Wash N Walk", according to SDS it has 1-5% of aminates. <br /> The contact with Morgan (Rudi Ramirez, 510-267-0134).Provided an update to <br /> Kasey Foley. <br /> 08/25/2015 1 entered the narrative for the incident PE 2546 SAEED <br /> SC 004 <br /> 08/25/2015 Chlorine gas: Prop 65 preparation PE 2546 MANZO <br /> SC 203 <br /> 08/25/2015 ER documentation concerning spill response. PE 2546 MANZO <br /> SC 544 <br /> 08/26/2015 Mailed Waste Discharge/Spill Response Notice to Abate dated 8/21/15 and photos PE 2546 BLACKWELL <br /> by certified mail to: MARTEN TRANSPORTATION, ATTN: NICHOLAS SC 177 <br /> SEGERSTROM, 129 MARTEN ST, MONDOVI WI 54755-1733. Certified mail# <br /> 9171 9690 0935 0084 8451 12. <br /> 08/26/2015 Reviewed an email received from David Peck on 8-22-2015 with bills of lading and PE 2546 MANZO <br /> SDS's for the disposed compounds. Saniwash (eco-san) has a pH of 12.5 and is SC 544 <br /> D002 (RCRA, corrosive, EPA registration No 1677-52), wash and walk SDS states <br /> that it will cause chlorine gas when mixed with bleach or chlorinated products. <br /> Page 3 of 4 <br />
