The following is an itemized list of hazardous waste violations that have not been <br /> addressed for ALEXANDERS DIESEL REPAIR as of Apri129, 2019. <br /> Open violations from March 04, 2019 inspection <br /> Violation #120 -Consolidated manifest receipts failed to contain complete information. <br /> Multiple consolidated manifests left by CleanTech Environmental Inc. were observed missing a signature of the <br /> generator, a signature of the hauler, the generator's EPA ID number, or the information on where the waste was <br /> going to. All copies of each receipt shall contain all of the following information: <br /> -The name, address, identification number, contact person, and telephone number of the generator, and the <br /> signature of the generator or the generator's representative; <br /> -The date of the shipment; <br /> -The manifest number; <br /> -The volume or quantity of each waste stream received, its California and RCRA waste codes, the waste stream <br /> type listed in subdivision (c), and its proper shipping description, including the hazardous class and United <br /> Nations/North America(UN/NA) identification number, if applicable; <br /> -The name, address, and identification number of the authorized facility to which the hazardous waste will be <br /> transported; <br /> -The transporter's name, address, and identification number; <br /> -The driver's signature; <br /> -A statement, signed by the generator, certifying that the generator has established a program to reduce the <br /> volume or quantity and toxicity of the hazardous waste to the degree, as determined by the generator, to be <br /> economically practicable. <br /> Ensure that all future receipts for hazardous waste shipments using a consolidated manifesting procedure contain <br /> all required information. <br /> Violation #121 -Failed to retain copy of manifest or bill of lading for spent lead acid batteries for 3 years. <br /> Bills of lading or manifests for the management of lead acid batteries for the last three years were not available for <br /> review during the inspection. The owner stated that the company that owns the property, Blackmun Equipment <br /> Leasing Co, manages the receipts. A copy of each bill of lading must be kept on site for at least three years. <br /> Immediately locate a copy of all bills of lading or manifests for lead acid batteries for the last three years, maintain <br /> them on site, and submit copies to the EHD. <br /> Violation #301 - Facility not maintained to minimize the release of a hazardous waste. <br /> Used oil was observed inside two small drains in the central work bay, one large drain in the north work bay, and <br /> inside a drain in the outside wash bay. The owner stated that he tries to prevent used oil from entering the two <br /> drains in the central work bay but some residual can leak into it. A stream of used oil was working its way towards <br /> a central work bay drain during the inspection. The owner was not sure if any of the drains lead to anywhere. <br /> Immediately stop all used oil from entering the drains. Also provide a piping layout of the all the drains in and <br /> around the business to show that no oil is going into a storm water system. If the system is contained, have the <br /> used oil pumped out and properly disposed of. <br /> Violation #403 -Failed to keep hazardous waste containers closed except when adding or removing <br /> hazardous waste. <br /> The following containers were observed open in the hazardous waste storage area: <br /> -One blue 55 gallon poly drum containing oily debris <br /> -One white 55 gallon poly drum containing used oil paper filters <br /> -One white 10 gallon drain pan containing used oil <br /> -Four black 55 gallon steel drums containing used oil <br /> -One black 55 gallon steel drums containing paper and metal used oil filters <br /> -Ten5 gallon buckets containing used oil <br /> All hazardous waste containers shall be closed at all times except when adding or removing waste. Immediately <br /> close these containers and ensure all hazardous waste containers are closed when not adding or removing waste. <br /> Page 3 of 5 <br />
