The following is an itemized list of hazardous waste violations that have not been <br /> addressed for ALEXANDERS DIESEL REPAIR as of Apri129, 2019. <br /> Open violations from March 04, 2019 inspection <br /> Violation #501 - Failed to completely label stationary hazardous waste tanks. <br /> One 270 gallon used oil tank was observed with a small smeared hazardous waste label blocked by a 55 gallon <br /> drum. <br /> One 110 gallon used antifreeze tank was observed without a hazardous waste label and an accumulation start date. <br /> All hazardous waste tanks shall be marked with the words"Hazardous Waste' and the accumulation start date. <br /> Immediately ensure that all hazardous waste tanks are marked with the required information. <br /> Violation #502 -Failed to comply with new hazardous waste tank system assessment requirements. <br /> Facility does not have a tank assessment for the 270 gallon used oil tank and the 110 gallon used antifreeze tank. <br /> The owner or operator of a hazardous waste tank shall obtain a written assessment reviewed and certified by an <br /> independent, qualified, professional engineer, registered in California attesting that the system has sufficient <br /> structural integrity, is acceptable for the transferring, storing, and treating of hazardous waste, and that the tanks <br /> and containment system are suitably designed. Immediately have an assessment prepared for the tanks. Submit a <br /> schedule showing when this will be accomplished to the EHD. <br /> Violation #507 -Failed to conduct daily inspections of hazardous waste tank system and maintain records <br /> onsite. <br /> Inspections of the hazardous waste tank system was not performed and documented. The following items are <br /> required to be inspected at least daily: <br /> 1. overfill/spill control equipment; <br /> 2. aboveground portions of the tank system to detect corrosion or releases of water; <br /> 3. data gathered from monitoring and leak-detection equipment; <br /> 4. outside areas of the tank system including secondary containment to detect erosion or signs of releases of <br /> hazardous waste; <br /> 5. for uncovered tanks, the level of waste in the tank. <br /> Immediately begin required inspections of the hazardous waste tank system, and document the items inspected. <br /> Submit a copy of the inspection log for the next 30 days to the EHD. <br /> Violation #601 -Stored hazardous waste on site longer than 90 days without a permit or authorization. <br /> The following containers/tanks in the hazardous waste storage area were observed without an accumulation start <br /> date in the hazardous waste accumulation area: <br /> -One blue 55 gallon poly drum containing oily debris <br /> -One white 55 gallon poly drum containing used oil paper filters <br /> -One black 55 gallon steel drum containing paper and metal used oil filter <br /> -Twenty-four old 5 gallon buckets containing a used oil <br /> - One 110 gallon used antifreeze tank <br /> Facilities who generate more than 1000 kg of hazardous waste per month may store waste on site up to 90 days. <br /> Since it cannot be determined how long the hazardous waste has been on site, immediately contact a licensed <br /> hazardous waste hauler to dispose of this waste under manifest. Submit a copy of the manifest to the EHD within 30 <br /> days. <br /> Page 4 of 5 <br />
