The hazardous waste training records and program on site are incomplete. Facility personnel shall <br />complete a training program in accordance with Title 22 regulations. This program shall be directed by a <br />person trained in hazardous waste management procedures and shall include the following: <br />- hazardous waste management procedures relevant to the positions in which they are employed <br />- contingency plan implementation <br />- emergency procedures, equipment, and systems <br />- key parameters for automatic waste feed cut-off systems <br />- communications or alarm systems <br />- response to fire, explosion, or groundwater contamination incident <br />- shut down of operations <br />Training records shall be maintained on site until closure of the facility and shall include: <br />- name & job title of each employee <br />- written job description for each position (job title) listed, including requisite skill, education, <br />qualifications, and duties <br />- description of training to be given to each position <br />- records that training was completed for each employee <br />Immediately prepare and implement a complete hazardous waste management training program for this <br />facility in accordance with Title 22 regulations and ensure that all employees complete this training at <br />least annually. Submit a copy of the training program and records the EHD by October 14, 2011. <br />REPEAT VIOLATION <br />122, 123, & 124. Manifest numbers 002042627FLE (10/2/08), 002042903FLE (11/05/09), and <br />007751423JJK (11/17/10) were found without a signed off copy from the destination facility. Hazardous <br />waste generators shall retain copies of all manifests signed off by the destination facility on site for three <br />years and have them readily available for review. If the generator did not receive a copy of the manifest <br />with the handwritten signature of the owner or operator of the facility to which the generator's waste was <br />submitted within 35 days of the date the waste was accepted by the initial transporter, the generator <br />shall contact the transporter and/or the owner or operator of the designated facility to determine the <br />status of the waste. If the generator did not receive a copy of the signed off copy of the manifest within <br />45 days of the date that the waste was accepted by the initial transporter, the generator shall submit an <br />Exception Report that includes a legible copy of the missing manifest, and a cover letter signed by the <br />generator explaining the efforts to locate the hazardous waste and the results of those efforts. <br />Exception reports shall be submitted to: <br />ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE. <br />THIS FACILITY IS SUBJECT TO REINSPECTION AT ANY TIME AT EHD'S CURRENT HOURLY RATE. <br />EHD Inspector: <br />Received By <br />Title: <br />Stacy ivera ) 468-3440 <br />E 0 CONTINUATION FORM <br />
