Box 806 <br />Sacramento, CA 95812-0806 <br />Immediately locate a copy of the missing manifests or prepare and submit an exception report to DTSC. <br />By October 14, 2011, submit to the EHD a copy of the signed off copy of the manifests or a copy of the <br />exception report submitted to DTSC. REPEAT VIOLATION <br />304. The eyewash and emergency shower at this facility was accidentally removed by a forklift and has <br />not been replaced. No other decontamination equipment was available on site. All facilities shall be <br />equipped with portable fire extinguishers, fire control equipment, spill control equipment, and <br />decontamination equipment as necessary, and shall be tested and maintained as necessary to assure <br />its proper operation in time of emergency. Immediately provide adequate decontamination equipment <br />for this and all other areas storing hazardous materials and wastes. REPEAT VIOLATION <br />605. The oil water tank was observed without a label. All hazardous waste tanks shall be marked with <br />the words "Hazardous Waste" and the accumulation start date. Immediately label this tank and ensure <br />that all hazardous waste tanks are marked with the required information. <br />605. Three 55 gallon black steel drums of waste water were observed with in the paint room with labels <br />stating only the contents and a planned pick up date of September 16, 2011. According to Mr. Lumpkin, <br />they were filled this week. All hazardous waste containers shall be marked with the following <br />information: <br />1. the words "Hazardous Waste" <br />2. name and address of generator <br />3. hazardous properties (flammable, corrosive, reactive, toxic) <br />4. physical state (liquid, solid) <br />5. composition (contents) <br />6. accumulation start date <br />Immediately label these containers and ensure that all containers are marked with all the required <br />information.
