The contingency plan for this facility did not include descriptions the phone number for the <br />California Emergency Management Agency (CalEMA), a list of emergency equipment at the facility, or <br />the name, address, and phone number of the facility Emergency Coordinator. A contingency plan shall <br />include all of the following: <br />-actions facility personnel shall take in response to fires, explosions, and unplanned release to air, <br />soil, or surface water <br />-arrangements agreed to by local police departments, fire departments, hospitals, contractors, and <br />state & local emergency response teams <br />- names, addresses & phone numbers (office & home) of the Emergency Coordinator(s) <br />-list of all emergency equipment at the facility (fire, spill, communications/alarms, decon), including <br />location, physical description, and a brief outline of its capabilities <br />-evacuation plan, including routes and signals to be used <br />-telephone number of Cal EMA <br />This information must be kept current at all times. Immediately update the contingency plan for this <br />facility to include all of the necessary information and submit a copy of the updated plan to the EHD by <br />October 14, 2011. <br />113. No emergency coordinator has been identified for this facility. At all times there shall be at least <br />one employee either on the facility premises or on call (i.e., available to respond to an emergency by <br />reaching the facility within a short period of time) with the responsibility for coordinating all emergency <br />response measures. This emergency coordinator shall be thoroughly familiar with all aspects of the <br />facility's contingency plan, all operations and activities at the facility, the location and characteristics of <br />waste handled, the location of all records within the facility, and the facility layout. In addition, this <br />person shall have the authority to commit the resources needed to carry out the contingency plan. <br />Immediately appoint an emergency coordinator for this facility. <br />133. A tank assessment was not available on site for the oily waste water tank. A hazardous waste <br />generator shall obtain and keep on site a written assessment for each hazardous waste tank at the <br />facility. This assessment shall be reviewed and certified by an independent, qualified, professional <br />engineer, registered in California, and shall attest to the tank system's integrity. The assessment shall <br />determine that the tank system is adequately designed and has sufficient structural strength and <br />compatibility with the waste stored, and shall be written in accordance with Title 22 regulations. <br />Immediately obtain a written tank assessment for the hazardous waste tank at this facility in accordance <br />ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE. <br />THIS FACILITY IS SUBJECT TO REINSPECTION AT ANY TIME AT END'S CURRENT HOURLY RATE. <br />EHD Inspector:Re <br />'v y: <br />Title: <br />Stacy era 09 68-3440 <br />T <br />EHD 23-02-003 ev 0 / 11 CONTINUATION FORM <br />
