R.W. Reade <br /> Page 2 <br /> As requested in correspondence from PHS/EHD on December 17, 1992, paragraph four (4), "the <br /> disposition of excavated soils is to be clarified". Please submit in an addendum the status of the soil in <br /> question. Include yardage excavated,yardage disposed of, destinations and receipts, as applicable. Soils <br /> (80 cubic yards) currently on site are inferred to be from the waste oil excavation and contaminated soils <br /> from the surface to 22 feet below ground surface near the "previously unknown fuel island". Clarify this <br /> assumption with figures and mathematical evidence to support your claim. If soils were brought onto the <br /> site, source and hauling records/receipts need to be submitted. December 17, 1992 correspondence from <br /> PHS/EHD, paragraph 6 requested laboratory data sheets (not consultant summary charts) for BTEX from <br /> 15 feet below ground surface at the waste oil tank. Include these sheets in your addendum along with <br /> appropriate chain of custody form(s). <br /> The following items were not included in The Health and Safety Plan: <br /> -name of emergency facility able to accommodate emergencies <br /> -address and phone number of facility <br /> -map of area with course to take from site to facility <br /> -names and phone numbers of contacts/key personnel in case of emergency <br /> Items deleted and addressed in previous paragraphs shall be submitted and approved prior to <br /> determination of this recently submitted workplan. <br /> You are hereby directed to submit all required items and answer all inquires addressed in an addendum to <br /> be submitted to PHS/EHD by July 17, 1992. <br /> Failure to comply may result in your site's inability to remain eligible for reimbursement under the "UST <br /> Cleanup Fund". <br /> Please address your inquires to Michael Infurna, Senior REHS, or call (209) 468-3454. <br /> Jogi Khanna, M.D., M.P.H. <br /> ealth Offiger <br /> Laurie A.
