Cherokee <br /> Lodi, CA 95241 <br /> We have reviewed and accepted your request for an extension to submit a Preliminary Assessment Report <br /> (PAR)by July 31, 1991,however,we request that a progress report be submitted by June 28,1991 outlining <br /> the work that has been completed to date. This progress report is to follow the requirements for your <br /> quarterly report. A quarterly report due in July can be incorporated into the PAR. <br /> We also have not received your quarterly monitoring report that was due to this office and to the Central <br /> Valley Regional Water Quality Control Board (CVRWQCB) by April 15, 1991. This report is past due and <br /> must be sent in immediately. <br /> The quarterly reports are to show the following: <br /> -status of investigation and cleanup activities including the results of all investigations completed <br /> to date; <br /> -proposed continuing or next phase of investigation; <br /> -method of cleanup proposed or implemented, schedules for the completion of the investigation of <br /> the site and remediation; <br /> -monitoring well data including groundwater elevation, PH, conductivity, gradient; <br /> -analytical results of groundwater or soil sampling; <br /> -a site map showing the "zero line" of contamination, if known, and changes in the above <br /> measurements over the last quarter; and <br /> -method and location of disposal of the contaminated soils or groundwater. <br /> (Include any manifest(s) and/or bills of lading.) <br /> If you have any questions, contact Eleanor Ratliff, REHS, at (209)468-3454. <br /> Jogi Khanna, M.D., M.P.H. <br /> Healyoffice ZHSPr <br /> fn <br /> e A.
