Reynolds Ranch APNs 050-10& -110 Page 2
Our Project Number: LESOa 578
November 5, 2008
Approximately five samples will be collected from the vicinity of the former underground
storage tank at locations to be determined by on-site personnel. These samples will be
collected at depths up to eight feet below ground surface (bgs).
2.2 Access Agreement
A signed access agreement is attached.
3.0 HEALTH AND SAFETY PLAN
We anticipate that the health hazards that will be encountered at the Site will be minimal.
Based on our understanding of the past disposal practices at the Site, we do not expect to
encounter any liquid or solid waste materials nor any liquid or solid hazardous waste materials
at the proposed drilling locations. However, if waste materials are encountered, we anticipate
that the primary mode of contaminant exposure will be through the skin as a result of handling
equipment, soil, and water. Therefore, we have prepared the following Health and Safety Plan:
1) Level D Protection. We propose that all field activities be conducted consistent
with Level D protection requirements as defined by the California Occupational Safety and
Health Administration (OSHA). The minimal required Level D protection equipment includes
hard hat, safety glasses, steel-toed boots, earplugs, disposable rubber gloves, and appropriate
work clothing.
2) Emergency Treatment. In the event of an emergency, a member of the team
will dial 911 and follow the instructions of the operator. In the event that a non-emergency
injury occurs to any or all of the team, they will be transported to the nearest hospital that
provides emergency room services (Lodi Memorial Hospital, 900 South Fairmont Avenue, Lodi,
209-333-2500).
4.0 SOIL SAMPLING PROCEDURES
4.1 Drilling Details
All drilling activities will be performed by NOA.
4.1.1 Drilling Method
The boring(s) in the vicinity of the former underground storage tank will be advanced with a 3-
or 4-inch diameter hand auger. The soil cuttings brought to the surface will be examined and
placed on plastic. Disposal will be based on the analytical results or field observations.
4.1.2 Boring Abandonment
All borings will be sealed and abandoned in accordance with EHD requirements. A
representative of the EHD may be present to approve all grouting activities.
'A4
902 Industrial Way•Lodi,CA 95240-209.367.3701 •Fax 209.369.4228 02007 Neil 0.Anderson&Associates,Inc
