Ms. Darlene Reuss
San Joaquin County Local Health District
September 7, 1989
Page 2
have approved, and that have been identified to protect the quality of ground water
monitored by each well. Prior to installation of the annular seal for these wells,
we will call you so that you may watch the completion of the seals if you desire.
We have identified an immediate need for three monitor wells on the
E. & J. Gallo property. Their approximate locations are shown on the enclosed
figure. Completed permit applications and the $105.00 application fee ($35.00 per
well) are enclosed. I would appreciate your returning the approved applications
to me rather than to P. C. Exploration. Would you also please send us ten (10)
additional blank permit forms.
I anticipate that by transmittal of this letter the process of acquiring
permits for construction of monitor wells will be made more efficient for you and
for LLNL. If there is any additional information which you would like us to send
you, please call Albert Lamarre at (415) 422-0757 or Fredric Hoffman at (415) 423-
6745.
Sincerely,
&-/-
Fredric Hoffman
Division Leader
Environmental Restoration Division
AQFH:dh
Distribution:
J.
