Chevron #9-4054 (former) page 2
2103 Country Club Blvd
After reviewing the details of this latest CAP revision, EHD approves of the equipment and
methods described to install and remove/destroy the vapor probes.
Based on Secor's evaluation of the soil vapor results, Chevron will "complete a focused
excavation to eliminate any vapor inhalation pathways if it is determined that natural attenuation
alone could not meet acceptable ground water clean up levels in a reasonable time frame."
The CAP revision is adequate and necessary and tentatively approved at this time [CA Health
& Safety Code 25296.1 0(c)(3)].
EHD will require that the new monitoring wells be added to the site's existing quarterly
monitoring and reporting requirements.
Since the proposed corrective action has been revised, public participation must again be
conducted. An Executive Summary is required that adequately addresses the latest CAP
revisions. Please modify one of the previously approved summaries to include the new
proposals and submit it to EHD by June 30, 2006.
After EHD has approved the Executive Summary, the site will be posted and a period of time
will be provided for the public to comment. After the comment period is completed, EHD will
advise you if a public hearing is to be scheduled. If a hearing has not been requested, EHD will
consider the CAP approved.
Submit two EHD Well Permit Application forms and two $89 fees to EHD by June 30,
2006. The offsite MW-9 cluster requires a San Joaquin County Public Works (SJC-PW)
encroachment permit before EHD can issue the monitoring well installation permit. Please
contact SJC-PW at (209) 468-3057 to obtain that agency's requirements for issuance of the
encroachment permit. A copy of the encroachment permit is to be submitted along with the
monitoring well installation permit application and fee to EHD.
Please contact Michael Infurna at (209) 468-3454 should you have any questions or wish to
discuss the site.
Donna Heran, REHS, Director
Environmental ,Health Department
Michael J.
