Chevron #9-4054 (former) page 2 <br /> 2103 Country Club Blvd <br /> After reviewing the details of this latest CAP revision, EHD approves of the equipment and <br /> methods described to install and remove/destroy the vapor probes. <br /> Based on Secor's evaluation of the soil vapor results, Chevron will "complete a focused <br /> excavation to eliminate any vapor inhalation pathways if it is determined that natural attenuation <br /> alone could not meet acceptable ground water clean up levels in a reasonable time frame." <br /> The CAP revision is adequate and necessary and tentatively approved at this time [CA Health <br /> & Safety Code 25296.1 0(c)(3)]. <br /> EHD will require that the new monitoring wells be added to the site's existing quarterly <br /> monitoring and reporting requirements. <br /> Since the proposed corrective action has been revised, public participation must again be <br /> conducted. An Executive Summary is required that adequately addresses the latest CAP <br /> revisions. Please modify one of the previously approved summaries to include the new <br /> proposals and submit it to EHD by June 30, 2006. <br /> After EHD has approved the Executive Summary, the site will be posted and a period of time <br /> will be provided for the public to comment. After the comment period is completed, EHD will <br /> advise you if a public hearing is to be scheduled. If a hearing has not been requested, EHD will <br /> consider the CAP approved. <br /> Submit two EHD Well Permit Application forms and two $89 fees to EHD by June 30, <br /> 2006. The offsite MW-9 cluster requires a San Joaquin County Public Works (SJC-PW) <br /> encroachment permit before EHD can issue the monitoring well installation permit. Please <br /> contact SJC-PW at (209) 468-3057 to obtain that agency's requirements for issuance of the <br /> encroachment permit.
