Prepare a draft and summary reports documenting the above <br /> activities and send four (4 ) copies to Owner c/o Neumiller & <br /> Beardslee, Post Office Box 20, Stockton, California 95201- <br /> 3020, Attention: Leah S. Goldberg; <br /> 7. Periodic sampling (and reporting) of soil and groundwater to <br /> check cleanup progress (per sampling interval) in accordance <br /> with PHS/EHD guidelines; <br /> 8. Verification sampling (post-closure monitoring) of groundwa- <br /> ter (per sampling interval) in accordance with PHS/EHD <br /> guidelines; and <br /> 9. For groundwater cleanup, the costs shall be assessed in <br /> terms of cost per gallon-per-minute (GPM) , and cost per <br /> 1, 000 gallons treated. <br /> D. Bidding Requirements <br /> Prospective consultants must be agreeable to or meet the follow- <br /> ing specifications: <br /> 1. The consultant must have at least five years experience con- <br /> ducting underground storage tank corrective action work in <br /> California. A statement of qualifications attesting to such <br /> experience must accompany each bid submitted. <br /> 2 . It is preferred, but not required, that the consultant have <br /> an office within 50 miles of the corrective action site. <br /> 3. The consultant must employ qualified persons to perform the <br /> work described above, including licensed geologists and <br /> other technical professionals that possess all necessary <br /> licenses to operate within the State of California and the <br /> jurisdiction of San Joaquin County. A list of qualified <br /> personnel who will perform the work must accompany your bid. <br /> 4 . The consultant must carry out the described work in a time- <br /> ly, professional, safe, and legal manner and maintain neces- <br /> sary records and other safeguards to ensure that reported <br /> sample results are accurate. All work must be performed in <br /> a manner acceptable to the FHS/EHD. <br /> 5. The consultant must provide at least three reference of pre- <br /> vious clients for whom the consultant has performed similar <br /> work within the past two years. <br /> 6. Consultant shall agree to enter a contract with Owner in a <br /> form substantially similar to the one attached hereto as At- <br /> tachment 2 . <br /> 7 . The consultant must maintain general liability insurance in <br /> the amount of $1, 000, 000.00 and professional errors and <br /> omissions liability insurance in the amount of $1,000, 000.00 <br /> for the life of the project.
