Spill Prevention, Control and Countermeasure Plan <br /> Tesla Treatment Facility Page 22 of 45 <br /> 2.5.3 Spill Coordinator <br /> All spills will be reported as soon as possible to the Chief Stationary Engineer. If another facility <br /> supervisor is provided with information regarding the spill, information as to the nature of the spill <br /> will be forwarded to the Chief Stationary Engineer who will then provide additional instructions <br /> to facility operators as-needed. <br /> The Chief Stationary Engineer will request the following information from the individual that <br /> discovered the spill: <br /> ® Name of the discoverer. <br /> ® Location of the discharge. <br /> ® Approximate volume of discharge. <br /> ® Direction of migration of the discharge. <br /> ® Action(s)being taken to stop and contain the discharge. <br /> 2.5.4 Spill Incident Report <br /> The Chief Stationary Engineer will begin the notification process as outlined in Section 2.4 of this <br /> SPCC Plan. The Chief Stationary Engineer or delegate will complete a spill incident report. <br /> Appendix E provides a spill incident report form. This written report will be provided to SFPUC <br /> WSTD within ten(10) days of the spill incident. <br /> 2.5.5 Spill Investigation <br /> The information gathered during a spill response helps to ensure that the proper resources and <br /> personnel are deployed and engaged in developing corrective actions to prevent future spills. <br /> Knowing what information to gather and who to notify is critical to ensure a fast effective response. <br /> The Chief Stationary Engineer, or alternate facility supervisor, shall be notified upon discovery of <br /> a spill. Facility operators are to only respond defensively if they had specific training on spill <br /> response and cleanup procedures. The Chief Stationary Engineer will notify facility operators, <br /> and any onsite contractors or visitors, if evacuation is necessary. <br /> • If a spill can be safely handled with available resources,the spill should be stopped and contained. <br /> • If there has been an offsite release,reporting of the spill is still required. <br /> • If the spill requires an emergency response, the Chief Stationary Engineer will notify SFPUC's <br /> WSTD and local emergency responder. Appropriate protocols in the facility's Hazardous <br /> Materials Business Plan will be implemented. <br />
