SAN10 A Q U I N Environmental Health Department <br /> C C_7 U '`J I Y <br /> Complaint Record Daily Time and Activity Summary from 1/1/2019 to <br /> 7/26/2019 for COQDPHYHI located at <br /> 15250 N THORNTON RD, LODI, CA <br /> Activity Date Recorded by <br /> 05/27/2019 1 PM - Received complaint from SJC General Hospital. Called and spoke to PE 2546 FLORIDO <br /> 342 min Candy of American Medical Response. A semi-trailer truck caught fire and SC 965 <br /> released an unknown amount of diesel, motor oil, and antifreeze into a drain. <br /> Called and spoke to Chief Butler of Woodbridge Fire. <br /> 1:42 PM -Arrived on site. Met with Woodbridge Fire's Eric Johnson (hazmat <br /> specialist), William Shaw(Love's Travel Stop Operations Manager), and Mike <br /> Swalley (Pat Fruth truck driver). Obtained incident information and responsible <br /> party information. Witnessed the damaged truck, chard debris in the parking lot, <br /> and oily sheen on both sides of the truck and leading to a drain in the parking lot. <br /> According to Woodbridge Fire, impacted drain leads to City of Lodi White Slough <br /> Water Treatment Facility. Responsible party (RP) is Pat Fruth Inc. Called and <br /> notified City of Lodi of impacted drain. Received a call back from the answering <br /> service for City of Lodi, who states because the drain is in an unincorporated area, <br /> City of Lodi would not be responding to the incident. Called and spoke to Jeff Sott <br /> of SJC Public Works who states he is familiar with the drainage system in the area <br /> and states the storm drains lead to a detention pond nearby. I told Jeff that the <br /> environmental contact for Love's, Jesse Diaz, says the drain leads to City of Lodi <br /> drainage system. <br /> 3 PM -Generated complaint, Notice To Abate, Emergency Response Record, <br /> Prop 65. Began documenting narrative. Called and spoke to Kevin Smith of Great <br /> Western Insurance and Rich Harness of LMH Environmental, both representatives <br /> on behalf of the RP. Kevin Smith and Ed Leitch, also of LMH, are en route. Called <br /> Jesse Diaz, who states the drain leads to a City of Lodi drain system, but he was <br /> unable to provide documentation to verify this information. <br /> 4:30 PM - Met with Ed Leitch of LMH and Kevin Smith of Great Western <br /> Insurance. Patriot Environmental en route ETA 1 hr. <br /> 4:45 PM - Met with Brandon of Patriot Environmental. Inspected the impacted <br /> drain, an other drain —250 feet west of first drain, two manholes that Ed believed <br /> lead to the oil water separator, and a manhole leading to a sanitary sewer line. <br /> The two drains appeared contaminated and diesel odor was coming from the <br /> drains. The three manholes appeared clean or free from contamination and there <br /> was no diesel odor coming from them. Unknown which way or where the impacted <br /> drain leads to. Patriot Environmental crew arrived on site. Checked refrigeration <br /> tank on truck and indicator shows it is 3/4 full. LMH and Patriot stated the two <br /> saddle tanks (125 gallons each, according to Woodbridge Fire) sounded empty or <br /> close to empty, upon knocking on the outside of each tank. The truck driver, Mike <br /> Swalley, had stated the saddle tanks were over half filled immediately prior to the <br /> incident. Called Fish and Wildlife to notify them that it is unknown where the <br /> impacted drain leads to, as I was first told it leads to City of Lodi White Slough <br /> Water Treatment. Fish and Wildlife stated if the incident was reported to Cal OES, <br /> Cal OES reports spills that impact waterways to Fish and Wildlife. Called Cal OES, <br /> they have not yet been notified. Told Ed Leitch of LMH, and provided Cal OES <br /> phone number. Ed called Rich of LMH and said Rich would notify Cal OES <br /> tomorrow morning. <br /> Called and discussed complaint with Muniappa Naidu, CUPA Program <br /> Coordinator. Revised Notice To Abate to include a violation for failure to <br /> immediately report any release or threatened release of a hazardous material to <br /> the California Governor's Office of Emergency Services, California State Warning <br /> Center, per Health and Safety Code 25510.
