Steel toed shoes or boots will be worn at all times when on the site.When possible, <br /> appropriate guards will be in place during equipment use. Field personnel should be careful to <br /> keep loose clothing, hands, and feet away from vacuum hose inlets. <br /> 3.3.4 Electrical Hazards <br /> In order to prevent accidents caused by electrical shock,field personnel will inspect electrical <br /> equipment being used (e.g. power tools, electrical cords) on a daily basis for evidence of frayed <br /> or loose connections. <br /> When necessary, electrical equipment is isolated in order to prevent the transmission or release <br /> of energy.This isolation for the control of hazardous energy is commonly called <br /> "Lockout/Tagout". If necessary, all provisions of the OSHA standard as stated in 29 CFR 1910.147 <br /> will be followed for controlling hazardous energy or locking out or tagging out electrical and <br /> utility hazards. <br /> 3.3.5 Lock-out/Tag-out <br /> Lock-out/Tag-out procedures should be used on any pieces of equipment being worked on by <br /> field personnel.The only exception to this is equipment with a plug, which can be unplugged <br /> and remain in control of the worker. Proper lock-out/tag-out includes the identification and <br /> isolation of hazardous energy.The energy sources must be locked out so that only the worker(s) <br /> has the key to release the isolation point. Stored energy(e.g. air or water pressure) must also be <br /> released prior to conducting work on the equipment. If necessary, all provisions of the <br /> Occupational Safety and Health Agency(OSHA)standard as stated in 29 Code of Federal <br /> Regulations (CFR) 1910.147 will be followed for controlling hazardous energy or locking out or <br /> tagging out electrical and utility hazards. <br /> 3.3.6 Confined Spaces <br /> Confined space entry is not anticipated for this project.Terraphase field personnel will not enter <br /> any confined space without specific approval of the Project Manager.Subcontractors may need <br /> to conduct permit-required confined space entries. In the event that a subcontractor project <br /> manager determines a permit-required confined space entry is necessary,the Terraphase <br /> Project Leader should be notified. <br /> If confined space entries are conducted they will be in accordance with the subcontractor's <br /> confined space entry program. An attendant will be stationed above the confined space while <br /> the entrant(s) are in the confined space.A full-body harness with emergency retrieval <br /> equipment or another means of egress (e.g. ladder)shall be in use if necessary. <br /> Before conducting any work inside a confined space (e.g.tank),the personnel will conduct a <br /> visual inspection of the area from the opening to identify any hazards before proceeding.
