0 0 <br /> • At no time will the operator of the drill rig operate the drill controls and add stem while alone, <br /> unless the drill rig is designed to operate as a single person operation; <br /> • Repairs and service must not be attempted while rotary machinery is running; <br /> • If electrical storms can be seen or heard, drilling operation will be shut down until it is deemed <br /> safe by the SSO. <br /> 2.2.3 Machinery/Moving Parts <br /> Drilling and vacuum equipment may have various motors, booms,and other equipment.These present a <br /> general physical hazard from moving parts. Personnel will stand clear of machinery at all times unless <br /> specific instructions are given by the trained operator,or other person in authority.Steel toed shoes or <br /> boots will be worn at all times when on the site. When possible,appropriate guards will be in place <br /> during equipment use. Field personnel should be careful to keep loose clothing, hands, and feet away <br /> from vacuum hose inlets. <br /> 2.2.4 Electrical Hazards <br /> In orderto prevent accidents caused by electrical shock,field personnel will inspect electrical equipment <br /> being used (e.g. power tools,electrical cords)on a daily basis for evidence of frayed or loose <br /> connections. <br /> 2.2.5 Slips,Trips, and Falls <br /> Slips,trips,and falls are a leading cause of injury on construction and remediation worksites. Water and <br /> slime collect on horizontal surfaces and stairs, posing a potential hazard. Proper housekeeping is the key <br /> to preventing injuries of this nature. <br /> • All potential trip and fall hazards will be clearly marked, modified to reduce the hazard, or <br /> engineered in a configuration to eliminate the hazard (if possible) <br /> • There will be no running on the site. <br /> As with all field work sites,caution will be exercised to prevent slips on rain slick surfaces, stepping on <br /> sharp objects, etc. Work will not be performed on elevated platforms without fall protection. <br /> 2.2.5.1 Hand Auguring <br /> • Avoid putting pressure on the palms of your hands; use a good grip to spread the pressure over <br /> the entire hand. <br /> • Take frequent stretch breaks to stretch and relax your back, arms and hands. <br /> 3.0 ROLES AND RESPONSIBILITIES <br /> The following section describes the roles and responsibilities of the personnel involved with the project. <br /> Terraphase Office: <br /> Office Number: (510) 645-1850 <br /> Corporate Health and Safety Officer: Daren Roth <br />
