2. PUBLIC HEALTH SERVICES: (Staff Contact: Greg Oliveira, 4684)341) �:✓ <br /> a. Submit four sets of detailed underground storage tank plants to the Environmental Health <br /> Division for review and approval prior to the beginning of construction. <br /> b. Submit an application for a hazardous waste permit to the Environmental Health Division. <br /> C. If floor drains are required, they must be plumbed through a sand/oil separator and <br /> discharged to an above-ground system approved by the Environmental Health Division. <br /> This system must be designed by a registered engineer. The material collected in the <br /> above-ground holding system must be analyzed for hazardous constituents prior to <br /> disposal. Specific test requirements may be obtained from the Environmental Health <br /> Division. A $156.00 plan check fee is required at the time of submittal. <br /> d. If hazardous material is stored on the site, the applicant must register with the County <br /> Office of Emergency Services. <br /> e. If hazardous waste is generated,the applicant must obtain an EPA identification number <br /> and comply with applicable regulations. <br /> 3. CITY OF STOCKTON DEPARTMENT OF PUBLIC WORKS: (Staff Contact: Bob Brothers, 937- <br /> 8374) <br /> a. Only one driveway access on Benjamin Holt Drive shall be allowed. The western-most <br /> driveway on Benjamin Holt Drive shall be abandoned and replaced with vertical face curb, <br /> gutter, and sidewalk. The eastern-most driveway may remain or be relocated westerly <br /> approximately 20 feet but shall conform to City of Stockton Standard Drawing No. 38, <br /> - modified to include 20-foot-radius curb retums. The maximum driveway width shall be <br /> 35 feet. <br /> b. Only one driveway access on Grigsby Place shall be allowed. The northern-most <br /> driveway on Grigsby Place shall be abandoned and replaced with vertical face curb, <br /> gutter, and sidewalk. The southern-most driveway may remain in its present location or <br /> relocated northerly approximately 40 feet. <br /> C. No signs or privately maintained structures of any kind may be placed within the public <br /> utility easement adjacent to the City's right-of-way on Benjamin Holt Drive. <br /> d. An encroachment permit, issued by this department, shall be required for all work within <br /> the City's right-of-way. Contact the City of Stockton Public Works Department at the City <br /> Permit Center(345 North EI Dorado Street,Stockton)for information regarding obtaining <br /> this permit. <br /> e. The right-of-way along Benjamin Holt Drive shall be dedicated when and as required by <br /> the City of Stockton upon determination of the final right-of-way alignment for the <br /> Benjamin Holt Drive/1-5 interchange improvements.
