2. PUBLIC HEALTH SERVICES: (Staff Contact: Greg Oliveira, 4684)341) �:✓ a. Submit four sets of detailed underground storage tank plants to the Environmental Health Division for review and approval prior to the beginning of construction. b. Submit an application for a hazardous waste permit to the Environmental Health Division. C. If floor drains are required, they must be plumbed through a sand/oil separator and discharged to an above-ground system approved by the Environmental Health Division. This system must be designed by a registered engineer. The material collected in the above-ground holding system must be analyzed for hazardous constituents prior to disposal. Specific test requirements may be obtained from the Environmental Health Division. A $156.00 plan check fee is required at the time of submittal. d. If hazardous material is stored on the site, the applicant must register with the County Office of Emergency Services. e. If hazardous waste is generated,the applicant must obtain an EPA identification number and comply with applicable regulations. 3. CITY OF STOCKTON DEPARTMENT OF PUBLIC WORKS: (Staff Contact: Bob Brothers, 937- 8374) a. Only one driveway access on Benjamin Holt Drive shall be allowed. The western-most driveway on Benjamin Holt Drive shall be abandoned and replaced with vertical face curb, gutter, and sidewalk. The eastern-most driveway may remain or be relocated westerly approximately 20 feet but shall conform to City of Stockton Standard Drawing No. 38, - modified to include 20-foot-radius curb retums. The maximum driveway width shall be 35 feet. b. Only one driveway access on Grigsby Place shall be allowed. The northern-most driveway on Grigsby Place shall be abandoned and replaced with vertical face curb, gutter, and sidewalk. The southern-most driveway may remain in its present location or relocated northerly approximately 40 feet. C. No signs or privately maintained structures of any kind may be placed within the public utility easement adjacent to the City's right-of-way on Benjamin Holt Drive. d. An encroachment permit, issued by this department, shall be required for all work within the City's right-of-way. Contact the City of Stockton Public Works Department at the City Permit Center(345 North EI Dorado Street,Stockton)for information regarding obtaining this permit. e. The right-of-way along Benjamin Holt Drive shall be dedicated when and as required by the City of Stockton upon determination of the final right-of-way alignment for the Benjamin Holt Drive/1-5 interchange improvements. It should be noted that the right-of- way drawings are presently being developed and should be completed within six months.
