The plans for the quarry excavation shall be revised to <br /> show existing and proposed improvement details:k of the <br /> entrance roads from Brandt Road and Cord Road. The first <br /> 100 feet of the entrance roads shall be paved to a 24- <br /> foot width. <br /> f. Cord Road shall be improved and widened to a minimum 20-- <br /> foot pavement width. The structural section is to be <br /> improved with an asphalt-concrete surface; the existing <br /> roadway is an oiled and chip-sealed surface. The bridges <br />} (#1570, #1572 , and W495) shall be replaced with concrete <br /> structures and designed to HS-20-Alternative and State <br /> Permit Loading. Plans shall be submitted for approval, <br /> and all applicable permits shall be acquired. <br /> g. A video tape of road conditions on Cord Road from the <br /> entrance road to State Route 12 is required prior to the <br /> use of Cord Road as a haul route. The conditions', of Cord <br /> Road from the site to State Route 12 shall be reviewed <br /> with the Department of Public Works prior to the use of <br /> Cord Road as a haul route. <br /> h. A bond or cash deposit in the sum of $10, 00011. 00, in <br /> addition to the existing $10, 000. 00 bond requirement for <br /> Brandt Road, shall be filed with the Department of Public <br /> Works to guarantee that if the Permittee fails, refuses, <br /> or neglects to repair, under permit, any damageslto Cord <br /> Road which may result from Permittee' s operations, the <br /> Department of Public Works may do the work necessary to <br /> restore Cord Road equal to, or better than, its original <br /> condition, using the funds from said bond or cash <br /> deposit. <br /> Conditions of Approval - 3 - (QX-86-i) <br />
