• 6.0 SITE DOCUMENTS <br /> 6.1 Environmental_Health Department Records <br /> Hazardous Materials Records <br /> The San Joaquin County Environmental Health Department was contacted <br /> regarding any hazardous materials records on file for the Site address of 6425 <br /> East Dougherty Road. According to Dom Martorella, no records or complaints <br /> are on file (correspondence, August 15, 2017). <br /> Well & Septic Permits <br /> Live Oak reviewed the San Joaquin County Environmental Health Department <br /> well and septic permit files for the Site addresses. One permit, dated April 1988, <br /> was identified. The permit was for a new septic system installation to serve a <br /> mobile home. The system included a 1,600-gallon septic tank, two 85-foot leach <br /> lines, and two 36-inch diameter by 25-foot deep seepage pits. Two wells are <br /> also depicted on the permit, which has been included in Appendix 4 of this <br /> report. <br /> • 7.0 GOVERNMENT AGENCY RESEARCH <br /> 7.1 EPA Databases <br /> The California Environmental Protection Agency (EPA) manages contaminated <br /> sites through two sub-agencies: the State Water Resources Control Board <br /> (SWRCB) and the Department of Toxic Substances Control (DTSC). The <br /> SWRCB's GeoTracker database includes underground storage tank (UST) sites, <br /> leaking underground storage tank (LUST) sites, cleanup program sites, and land <br /> disposal sites. The database can be searched by reviewing two lists, the <br /> Permitted Underground Storage Tank (UST) Sites list and the Cleanup Sites list. <br /> DTSC's EnviroStor database includes Federal Superfund sites, State response <br /> sites, voluntary cleanup sites, school cleanup sites, corrective action sites, and <br /> permitted hazardous waste facilities. <br /> Live Oak searched both databases on July 25, 2017 for any listed sites within <br /> one-quarter mile of the Site. The subject Site was not listed on either database, <br /> and no facilities were identified within one-quarter mile. <br /> • <br /> LOGE 1720 5 <br />
