6.4 SITE DOCUMENTS <br /> 6.1 Environmental Health Department Records <br /> Live Oak requested copies of the EHD well and septic permit files for the Site addresses <br /> of 24242, 24650, and 24688 north Mackville Road. According to Cheryl Field of that <br /> office, no permits are on file for those addresses (Correspondence, September 23, <br /> 2010; Personal Communication October 4, 2010)_ Ms. Field located one septic permit <br /> for the Site under the address of 24658 Mackville Road. This permit, dated 1999, is for <br /> the addition of one 40-foot leach line and one 36-inch diameter by 25-foot deep <br /> seepage pit to an existing residential system. Mr. Henry indicated this permit must <br /> refer to the stone house since the wooden house has not been occupied recently. <br /> The EHD was contacted regarding the Site addresses of 24242, 24650, 24658, and <br /> 24688 North Mackville Road. According to Cheryl Field (Personal Communication, <br /> October 4, 2010) and Evette Johnson (Personal Communication, October 11, 2010), no <br /> records related to environmental issues are on file for the Site addresses. <br /> 7.0 GOVERNMENT AGENCY RESEARCH <br /> 7.1 EPA Databases 6? <br /> The California Environmental Protection Agency (EPA) manages contaminated sites <br /> through two sub-agencies: the State Water Resources Control Board (SWRCB) and the <br /> Department of Toxic Substances Control (DTSC). The SWRCB's GeoTracker database <br /> includes underground storage tank (UST) sites, leaking underground storage tank <br /> (LUST) sites, cleanup program sites, and land disposal sites. DTSC's EnviroStor <br /> database includes Federal Superfund sites, State response sites, voluntary cleanup <br /> sites, school cleanup sites, corrective action sites, and permitted hazardous waste <br /> facilities. <br /> Live Oak searched both databases on September 24, 2010 for any listed sites within <br /> one-quarter mile of the Site. The subject Site was not listed on either database, and no Com. <br /> listed facilities were identified within one-quarter mile of the Site. <br /> 7.2 Environmental Health Department Databases <br /> On October 5, 2010, Live Oak reviewed two databases maintained by the EHD, the <br /> Underground Storage Tank Site Mitigation List, dated May 5, 2010, and the Unit !V <br /> Sites (LOP) list, dated.
