L <br /> L September 30,2005 <br /> NOA Project Number: E05120B <br /> i <br /> L 1.3 Access Agreement <br /> Prior to performing work for this investigation,NOA will obtain a written access agreement from <br /> �. the owner of the Site. The agreement will be executed between NCA and the owner and will allow <br /> NOA and all subcontractors access for on-site activities required for this investigation. <br /> L, 1.4 Underground Service Alert <br /> ` NOA will notify Underground Service Alert to clear the proposed drilling locations at least 48 hours <br /> ` prior to commencement of any subsurface fieldwork,as required by law. <br /> 2.0 HEALTH AND SAFETY PLAN <br /> We anticipate that the health hazards that will be encountered at the Site will be minimal.Based on <br /> our understanding of the past disposal practices at the Site,we do not expect to encounter any liquid <br /> ` or solid waste materials nor any liquid or solid hazardous waste materials at the proposed drilling <br /> locations. However,if waste materials are encountered,we anticipate that the primary mode of <br /> contaminant exposure will be through the skin as a result of handling equipment,soil,and water. <br /> Therefore,we have prepared the following Health and Safety Plan: <br /> r <br /> 1) Level D Protection. We propose that all field activities be conducted consistent with <br /> Level D protection requirements as defined by the California Occupational Safety <br /> and Health Administration(OSHA). The minimal required Level D protection <br /> equipment includes hard hat,safety glasses,steel-toed boots,earplugs,disposable <br /> rubber gloves,and appropriate work clothing. <br /> 2) Emergency Treatment. In the event of an emergency,a member of the team will dial <br /> 911 and follow the instructions of the operator. In the event that a non-emergency <br /> b. injury occurs to any or all of the team,they will be transported to the nearest hospital <br /> that provides emergency room services (Sutter Tracy Community Hospital,1420 N. <br /> Tracy Blvd.,Tracy,209-835-1500). <br /> i <br /> 3.0 SOIL SAMPLING PROCEDURES <br /> 3.1 Quality Assurance/Quality Control (QA/QC) Procedures <br /> Quality Assurance/Quality Control(QA/QC) procedures will be used to minimize the potential for <br /> contamination of soil samples. The QA/QC procedures may include the use of single-use sampling <br /> equipment,decontaminated of drilling and sampling equipment by washing with a solution of <br /> potable water and Liquid-Nox solution or equivalent between borings,use of one duplicate sample <br /> L per day,changing latex protective gloves after each sample,and recording sample information on <br /> chain-of-custody forms. <br /> L 3 <br /> i <br />
