f . The access road and circulation shall coordinate with the <br /> Lathrop Industrial Park development (SU-87-25/Kearny <br /> Ventures) . The access road shall be a minimum of 20-feet <br /> wide with the structural section and grade adequate to <br /> provide ultimate design gravel equivalent of our standard <br /> for an 84-foot industrial street after widening and adding <br /> a two-inch overlay. <br /> 3 . Local Health District : <br /> a. Connection to an approved water system is required. <br /> b . Connection to a public sewer system is required. <br /> c. Cake Material : A waste content determination must be made <br /> prior to disposal . Contact DOHS-TSCD to verify who will <br /> make the determination and where waste can ultimately be <br /> disposed. <br /> d. An application must be made to the San Joaquin County Air <br /> Pollution Control District for the necessary air pollution <br /> permit . <br /> e . File a Report of Waste Discharge with the California <br /> Regional Water Quality Control Board, Central Valley <br /> Region. <br /> f . Obtain an operating permit from the Federal Environmental <br /> Protection Agency. <br /> gig. Prior to construction, provide the San Joaquin Local <br /> Health District with copies of all documents and reports <br /> sent to the California Regional Water Quality Control <br /> Board and the Environmental Protection Agency. <br /> ,"rior to construction, obtain an injection well permit <br /> from the Health District after approval by the California <br /> Regional Water Quality Control Board and the Environmental <br /> Protection Agency. <br /> i . A cement grout seal will be required in the annular space <br /> between the outer conductor casing and the well casing to <br /> an impervious formation below 100 feet . <br /> 4 . Office of Emergency Services: <br /> The facility shall comply with Chapter 6 . 95 , Section 25500 , <br /> et seq . , of the California Health and Safety Code . Prior to <br /> start-up of the facility, the applicant shall submit an <br /> Emergency Plan and a Risk Management and Prevention Plan to <br /> the Office of Emergency Services . A copy of each shall also <br /> be submitted to the Planning Division. <br /> 5 . Air Pollution Control District : <br /> Conditions of Approval - 3 - (UP-89-22 ) <br />
