An Environmental Questionnaire and <br /> i Disclosure Statement was completed by Mr.Larry Johnson,Program Director of STAND,on October <br /> L 7, 2005. A copy of his responses has been included in this report as Appendix E. /2 <br /> In his responses to the questionnaire,Mr.Johnson indicated that he has no specialized knowledge of l <br /> L environmental conditions at the subject property and that there is no additional purpose for the SSCR <br /> beyond satisfying San Joaquin County requirements for a parcel division or lot line adjustment. <br /> �. Mr. Charles Ellis, pervious owner was not interviewed. Mr. Larry Johnson mailed a questionaire <br /> provided by NOA to the escrow company for the previous owners to complete. As of the date of this <br /> L report, the questionnaire has not been returned. Mr. Larry Johnson was contacted on November 1, <br /> 2005 in order to obtain the address or phone number for Mr.or Mrs.Ellis. Mr.Larry Johnson referred <br /> me to the escrow company stating that he does not have that information. The escrow company was <br /> y contacted on November 1, 2005,the escrow office for this transaction would not provide the names <br /> or contact information for the previous owners as this is confidential information. The assessor parcel <br /> information was retrieved by NOA for the subject property. According to the parcel information the <br /> p previous owners are Charles and Terry Ellis. With a mailing address of 28133 23' street, Hayward, <br /> L California 94544. An online phone number search was conducted for Ellis, T Ellis, and C Ellis in <br /> Hayward,California. One phone number was found for a Terry Ellis at 28133 E. 12'Street,Hayward, <br /> California. Several attempts have been made to contact this person. No contact has been made as the <br /> ti. date of this report. <br /> 3.0 PAST AND CURRENT SOURCES OF CONTAMINATION <br /> t 3.1 Research Methods <br /> Ir NOA employs a standard practice for review of database information. On a street map showing the <br /> location of the subject property, a circle is drawn one-quarter mile out from the property boundaries, <br /> and street maps or web-based mapping services are then used to determine address ranges for all streets <br /> L within the quarter mile.
