SAN 10 A Q U I N Environmental Health Department
t U "d I V
Complaint Record Daily Time and Activity Summary from6/21/2013 to
6/21/2020 for C00050437 located at
3019 NAVY DR
Activity Date Recorded by
09/06/2019 3:30 PM - Notified by Elena Manzo, Ld. Sr. REHS, of the complaint information. PE 2546 FLORIDO
90 min Prepared for complaint inspection and left SJC EHD to relieve Claudia Muro, SC 544
EHST, on site.
4:00 PM -Arrived on site. Met with Claudia Muro, EHST, Ridwan Sayyed and
Chuck Struble of WBCM Construction (responsible party), and Michael Anderson
of Yara North America. Observed and smelled what appeared to be standing
diesel fuel at the bottom of a trench, approximately 7 feet in depth.
The diesel appeared to have come out of the steel pipe at the bottom of the
trench, which contained the auger drill used by WBCM Construction Services to
install a liquid fertilizer pipe, on behalf of Yara North America. WBCM marked the
location on the ground in which they state the drill struck what they think may be
an abandoned fuel line, —7 feet beneath the ground. The distance from the WBCM
mark on the ground to the location of the observed fuel in the trench is 65 -80 feet
long. WBCM has contracted Ancon Services to conduct an excavation to reveal
the source of the release, using "hydro excavation," initially scheduled to begin
tomorrow morning.
4:40 PM -Called and updated Elena Manzo, Ld Sr REHS. Elena voiced her
concern regarding the use of water to excavate, and how it may further
contaminate the soil and possibly the water table.
4:56 PM -Called and updated Muniappa Naidu, CUPA Program Coordinator.
5:00 PM - END.
09/09/2019 Visited site with Muniappa. Met with Mark Bare, Engineering Geologist for PE 2546 FLORIDO
60 min Regional Water Quality Control Board, Michael Anderson of Yara North America, SC 544
Sayyed Ridwan and Chuck Struble of WBCM Construction Services. Ridwan
stated the excavation would be done using a mini excavator and by hand digging,
rather than the hydro excavation. Mark confirmed that he would oversee the
clean-up going forward, and Sayyed to keep me CC'd in any emails sent regarding
the clean-up.
09/09/2019 Revised complaint information in Envision Connect. PE 2546 FLORIDO
36 min SC 544
Generated Prop 65 and Emergency Response Record.
09/10/2019 Generated photo log. Generated agency referral letter to refer the complaint to: PE 2546 FLORIDO
72 min Mark Bare, Engineering Geologist SC 544
Central Valley Regional Water Quality Control Board
11020 Sun Center Drive, Suite 200
Rancho Cordova, CA 95670
Office: (916) 464-4820
Fax: (916)464-4645
Email: Mark.Bare@waterboards.ca.gov
Printed and organized complaint documentation.
