SAN J O A Q U I N Environmental Health Department <br /> Complaint Record Daily Time and Activity Summary from6/21/2013 to <br /> 6/21/2020 for C00050437 located at <br /> 3019 NAVY DR <br /> Activity Date Recorded by <br /> I responded to Sayyed: "Ridwan, I understand. After discussion with my superior, <br /> we would like to not rush into making a decision as to whether or not your plan is <br /> acceptable. We will reach out to the State Waterboard and let you know on <br /> Monday." <br /> I also received an email, which Ridwan sent to Jack Harrah, Cal OES Sr <br /> Emergency Services Coordinator, to report the spill incident. I notified Ridwan that <br /> he needs to call 1-800-852-7550 to report the spill to Cal OES. Ridwan provided <br /> me with the Cal OES report#19-5783. <br /> Entered complaint information into Envision Connect Remote, in order to generate <br /> the Prop 65 and Emergency Response Record form. Generated Notice To Abate, <br /> however I did not enter the responsible party mailing address. Emailed Sayyed <br /> Ridwan requesting he confirm the responsibly party name, mailing address, and <br /> billing address. Emailed Sayyed Ridwan the Notice To Abate without the <br /> responsible party mailing address. Documented complaint narrative. <br /> 7:30 PM - END. <br /> 7:48 PM - Received a page. Called SJC General Hospital and was notified that <br /> Armando Aroyo (916-862-2968) of the California Public Health Department <br /> (CDPH) reported a hazardous material spill of an unknown chemical and quantity <br /> and an unknown address. Called Armando, but was unable to leave him a voice <br /> message because his voice mail box was full. Text Armando from the EHD ER <br /> cell phone requesting that he call me back to discuss the spill complaint. Armando <br /> called me and stated he was the CDPH Duty Officer on-call and was on the other <br /> line when I called him. He stated the Cal OES report# is 19-5783 and reports a <br /> fuel spill due to a boring operation. Armando confirmed the incident occurred at <br /> 3019 Navy Dr, Notified Armando that I had received this complaint earlier and <br /> visited the site. <br /> 8:00 PM - END. <br /> Page 2 of 7 <br />
