Manifest <br />number 009912504FLE (11/11/16) was not found on the DTSC HWTS website. Mr. Bishop stated that they do not <br />mail copies of any manifests to DTSC. The generator manifest copy shall be submitted to DTSC for every shipment <br />on a manifest when California is either the generator state or the destination or consignment state. The generator <br />manifest copy shall be mailed to: <br />DTSC Generator Manifests <br />P.O. Box 400 <br />Sacramento, CA 95812-0400 <br />Immediately mail a copy of the manifests listed and ensure that copies of all generator manifests are sent to DTSC <br />within 30 days of shipping hazardous waste. <br />This is a Class II violation. <br />301 <br />CCR 66265.31 Facility not maintained to minimize the release of a hazardous waste. <br />Oily staining was observed in a large area on the blacktop between the building and the treatment unit. According to <br />Mr. Bishop, the staining is caused by a large piece of equipment that leaks. No drip pans or containment was <br />observed under the leaking equipment. Facilities shall be maintained and operated to minimize the possibility of a <br />fire, explosion, or release of hazardous waste to air, soil, or surface water which could threaten human health or the <br />environment. Immediately clean all oily drip areas, take all necessary steps to prevent additional leakage to the <br />ground, and manage wastes generated according to Title 22 hazardous waste regulations. Submit a statement and <br />supporting documentation explaining how the waste was managed. <br />This is a repeat violation, Class II. <br />303 <br />CCR 66265.33 Failed to maintain all communication or alarm systems, spill control, or decontamination equipment. <br />The north eyewash shower at the truck wash area would not come on when the handle was pulled. The <br />maintenance worker on site inspected the shower and stated it was a faulty valve. The eyewashes and south <br />shower were demonstrated as working. All facility communications or alarm systems, fire protection equipment, <br />spill control equipment, and decontamination equipment, where required, shall be tested and maintained as <br />necessary to assure its proper operation in time of emergency. Immediately begin this practice. <br />This is a Class II violation. <br />502 <br />CCR 66265.192(x) Failed to comply with new hazardous waste tank system assessment requirements. <br />Facility does not have a tank assessment for the used oil tank. The owner or operator of a hazardous waste tank <br />shall obtain a written assessment reviewed and certified by an independent, qualified, professional engineer, <br />registered in California attesting that the system has sufficient structural integrity, is acceptable for the transferring, <br />storing, and treating of hazardous waste, and that the tanks and containment system are suitably designed. <br />Immediately have an assessment prepared for the used oil tank.
