The permit has been approved with the <br />following conditions: <br />Contact the local fire department and the San Joaquin Valley Air Pollution Control <br />District for any applicable permits and requirements. <br />2. Any changes in information and scope of work must be submitted to EHD for approval <br />prior to commencing work. All new addenda supersede the previous ones. <br />3. Contact Jeffrey Wong at (209) 468-0335 to arrange the following inspection <br />appointments (a 48-hour notice is required for inspection appointments). Please be <br />aware that inspections for emergency work must be scheduled within five days of permit <br />approval. <br />a. Positive Shutdown at dispenser and 87 -octane turbine sump: Turbine shutdown or <br />suspension of fuel must be verified to ensure that positive shutdown for both 87 - <br />octane has been restored to meet the requirements of Title 23 California Code of <br />Regulations and the Health & Safety Code. <br />b. Leak detector test: Leak detector must be tested to confirm that the device can <br />detect a leak of 3 gallons per hour at 10 psi. <br />Please be aware that the facility cannot operate the UST system prior to the EHD <br />inspection and approval. <br />4. Any individual performing the work of a service technician must meet the following <br />qualifications: <br />a. Possess or be employed by a person who possesses one of several permissible <br />valid, current contractor's licenses issued by the Contractors State License Board <br />
