PESCADERO AVENUE <br /> Activity Date Recorded by <br /> 08/09/2019 At 10:15 am called Derek Smith with Patriot Environmental at 209-916-919-3620 PE 2546 BAKER <br /> 150 min to confirm that the clean-up would be completed at 12:00pm. Derek Smith said SC 544 <br /> that the clean-up was completed already and that I could come out and verify. <br /> Arrived on site and met with Derek Smith of Patriot Environmental. He took me to <br /> the clean-up area. Clean-up was complete. No storm drains were impacted by <br /> the clean-up process. The closest storm drain had a berm around it. All bays that <br /> were impacted by the fire were power washed including the bays immediately to <br /> the left and right of the impacted area. Five 55 gallon drums of rinse water, two <br /> roll off bins of fire debris, three 55 gallon drums of oily fire debris and three 55 <br /> gallon drums of aerosol can fire debris were generated. All waste will remain on <br /> site pending waste profiling and analytical of the fire debris waste in the roll off <br /> bins. Two samples were taken of the roll off bin waste. <br /> Completed photo log. <br /> 08/09/2019 Provided complaint information to Lydia Baker, REHS, who is on C today. She is PE 2546 FLORIDO <br /> 114 min going to verify the clean-up is completed today. SC 544 <br /> Generated Notice To Abate and emailed to Jaime Haines of YRC. Generated Prop <br /> 65 (Notification of Hazardous Waste Discharge). Generated Emergency <br /> Response Record, including site map. <br /> Printed and gathered complaint documentation. <br /> Documented yesterday's narrative. <br /> Received the following spill clean-up update from Lydia: "Clean-up is done. Two <br /> roll-offs of fire debris waste, three 55 gallon drums of oily fire debris waste, three <br /> 55 gallon drums of aerosol can fire debris waste and five 55 gallon drums of the <br /> rinse/power washing water. All the waste is staying on site until they get the <br /> analytical and waste profiling done. Two samples were taken for the fire debris <br /> waste. The drains were ok. They still had the berm things around them. The <br /> photo log is attached." <br /> 09/06/2019 Reviewed narrative. Emailed Jamie Haines of YRC Freight the following: "At your PE 2546 FLORIDO <br /> 18 min earliest opportunity, please provide the sample analytical results for the fire debris SC 544 <br /> waste, as well as the final hazardous waste manifest/disposal records for all of the <br /> waste generated as a result of the YRC trailer fire on 8/8/19. <br /> Please note that all my time spent on abating this complaint will be billed to the <br /> responsible party at our agency's current rate of$152/hour. <br /> Feel free to contact me if you have any questions." <br /> Page 2 of 4 <br />
