REPEAT VIOLATION <br /> 703. An emergency coordinator and modified contingency plan information is lacking. There must be at <br /> least one emergency coordinator on site or on call to coordinate emergency response measures, and <br /> the following information must be posted by a phone: the name and phone number of the emergency <br /> coordinator; location of fire extinguishers, spill control equipment, and if present, fire alarm; and the <br /> phone number of the fire department, unless the facility has a direct alarm. Immediately appoint an <br /> emergency coordinator and post the required information by a phone. A form is provided that can be <br /> used for this purpose. Submit proof of correction to the EHD by October 20, 2011. REPEAT <br /> VIOLATION <br /> Notice to Comply - Minor violations: (611, 809) <br /> 611. A 55 gallon drum of drained used oil filters was observed in the hazardous waste storage building <br /> with an accumulation start date of May 20, 2010. Facilities that generate up to one ton of used oil filters <br /> per year may store the filters up to one year. Immediately recycle these filters under a bill of lading and <br /> submit a copy of the bill of lading to the EHD by October 20, 2011. <br /> 809. A 5 gallon bucket of universal waste batteries was observed in the maintenance shop with an <br /> accumulation start date of October 22, 2009. The facility was unable to demonstrate the amount of time <br /> used fluorescent bulbs, electronic wastes, and aerosol cans were stored on site. A generator of <br /> universal waste (alkaline batteries, mercury containing equipment, fluorescent bulbs, electronic devices, <br /> CRTs, aerosol cans) shall accumulate universal waste for no longer than one year from the date the <br /> waste was generated.
