SAN JOAQUIN COUNTY <br /> ENVIRONMENTAL HEALTH DEPARTMENT <br /> 600 East Main Street, Stockton, California 95202-3029 <br /> Telephone:(209)468-3420 Fax:(209)468-3433 Web:www.sjgov.org/ehd/unitiii.html <br /> CONTINUATION FORM Page: 6 of 7 <br /> OFFICIAL INSPECTION REPORT Date: 09/20/11 <br /> Facility Address: 1051 Sperry Rd., Stockton Program: HW <br /> SUMMARY OF VIOLATIONS <br /> CLASS 1,CLASS II,or MINOR-Notice to Comply) <br /> Complete and submit a copy of the Return to Compliance Certification to the EHD by October 20, 2011 <br /> along with a statement documenting the corrective actions that have been or will be taken for each <br /> violation and any supporting paperwork. <br /> "Facility generated 3.48 tons of hazardous waste in 2010. <br /> Notes: (corrected on site) <br /> - Two manifests were found on site at the facility but were not on record with the Department of Toxic <br /> Substances Control (DTSC). Within 30 days of each shipment, a generator shall submit a legible copy <br /> of each manifest used to DTSC. A copy of each manifest was mailed at the time of inspection. <br /> - Two manifests were not found on site. Two manifests were found without a signed off copy from the <br /> destination facility. Hazardous waste generators shall retain copies of all manifests for three years or <br /> until a copy signed off by the destination facility is received. The signed off copy shall be retained and <br /> made readily available for review for three years. If the generator did not receive a copy of the manifest <br /> with the handwritten signature of the owner or operator of the facility to which the generator's waste was <br /> submitted within 60 days of the date the waste was accepted by the initial transporter, the generator <br /> shall submit a legible copy of the missing manifest, with some indication that the generator has not <br /> received confirmation of delivery. This information shall be submitted to: <br /> DTSC Report Repository <br /> Generator Information Services Section <br /> P.O. Box 806 <br /> Sacramento, CA 95812-0806 <br /> The missing manifests were faxed from the destination facilities at the time of inspection. <br /> - Used fluorescent bulbs were observed stored open and uncontainerized in the maintenance shop. <br /> Universal waste lamps shall be contained in a closed container or package that is structurally sound, <br /> adequate to prevent breakage, and compatible with the contents of the lamp.
