#RX00040-2011 <br />03/16/11 <br />❑ The receptacle will initially be set up by the StrongPak representative. <br />➢ The receptacle will be lined. <br />➢ The StrongPak representative will leave additional liners in the event they are <br />needed in between visits. <br />❑ Place the designated receptacle in the pharmacy in an area out of view of customers. <br />❑ Document store information and date in the areas on the card located inside the sleeve on <br />the front of the receptacle. <br />➢ The date on the card identifies when the accumulation of P -Listed waste began. <br />❑ Begin placing waste into the designated receptacle. <br />➢ Any PHI must be de -identified. <br />➢ Any returned to stock product that is not in the original packaging, past expiration <br />date and has no value are considered waste and should be placed inside the <br />receptacle. This includes OTC products used in the pharmacy. <br />Certain items should be placed inside a sealable baggie prior to being placed inside <br />the receptacle including: <br />■ Warfarin or Coumadin Stock Bottles or prescription vials <br />■ Any loose non -controlled substance tablets <br />• Damaged Nicotine products <br />■ Waste generated from compounds <br />❑ Only open the receptacle to deposit waste. Maintain closed when not in use. <br />❑ A StrongPak representative will visit your store on a weekly basis to inspect and evaluate <br />and get your feedback on the process. <br />❑ In between visits, if the receptacle becomes full: <br />➢ Remove the liner and tie off. <br />➢ Keep the sealed liner in the pharmacy area until the StrongPak representative <br />returns. <br />❑ StrongPak will coordinate a collection pick up (frequency will be based on the inspection <br />process). <br />❑ During the pick-up, Strong Pak will: <br />➢ Create the inventory log of waste in the receptacle and generate a manifest. <br />➢ Create a Pharmaceutical verification form to be signed by the pharmacist. <br />➢ Retrieve and remove the waste. <br />➢ Provide a new card to be placed in the front sleeve of the receptacle to identify when <br />accumulation of P -Listed waste began. <br />Evaluate and inventory Phentermine waste to report to the corporate office. <br />
