It could not be demonstrated how long the <br />hazardous waste had been on site and the Store Manager said they have had no waste hauled offsite in over 6 months. <br />Immediately have these wastes removed by a registered hazardous waste hauler, under manifest, and submit <br />a copy to this office. <br />ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE ($122). <br />THIS FACILITY IS SUBJECT TO REINSPECTION AT ANY TIME AT D'S CURRENT HOURLY RATE. <br />EHD Inspec <br />Rec iv <br />Titl <br />EHD 2 2-003 Rev 10/10 CONTINUATION FORM <br />
