Assuring that personnel are aware of the provisions of this plan, are <br /> instructed in the work practices necessary to ensure safety, and in <br /> planned procedures for dealing with emergencies. <br /> S. Assuring that personnel are aware of the potential hazards associated <br /> with site operations. <br /> 4. Monitoring the safety performance of all personnel to ensure that the <br /> required work practices are employed. <br /> S. Correcting any work practices or conditions that may result in injury or <br /> exposure to hazardous substances. <br /> 6. Preparing any accident/inc ident reports (see attached Accident Report. <br /> Form). <br /> 1.4 Project Enployee <br /> Project Employees involved in on-site investigations are responsible for: <br /> 1. Taking all reasonable precautions to prevent injury to themselves and to <br /> their fellow employees. <br /> 2. Implementing Health and Safety Program and reporting to the PSO for <br /> any action deviating from the anticipated conditions described in the <br /> Health and Safety Plan. <br /> 1.5 Special1mvtructions <br /> It is the responsibility of the Project Manager and the Health and Safety <br /> Manager to review the available site information to determine, to what degree <br /> the Health and Safety Plan will be implemented. Each site will be handled on a <br /> case by case basis. This site, because of the nature and known levels of <br /> contaminates, has been identified as a Level "D" entry. Based on U.S. EPA <br /> rating criteria for entry, this site requires a minimum of protection. This will <br /> include rubber boots, tyvek, gloves, and hard hats. <br />
