24 , 1986 , he retained the services of SEMCO general Engineers <br /> of Modesto to remove three ( 3) gasoline underground storage <br /> tanks, two of which had a 2 ,000 gallon capacity, and the third <br /> one a 3, 000 gallon capacity. Also removed from the site was a <br /> 350 gallon waste oil UST. A copy of the approved removal plan <br /> for these USTs is attached hereto as Exhibit "A" . At the time <br /> the four (4) USTs were removed, an inspection report was <br /> prepared by the PHS/EHD' s predecessor in interest, the San <br /> Joaquin Local Health District, a copy of that report is <br /> attached as Exhibit "B" hereto. Also attached as Exhibits "C-1" <br /> though "C-41' . inclusive, are the UST Permit Application - Form <br /> B. <br /> On or about March 7 , 1994, Mr. Tsirelas received a letter <br /> from PHS/EHD apprising him of possible contamination at the <br /> site based upon soil and water tests done at the site at the <br /> time the USTs were removed. A copy of the letter is attached <br /> hereto as Exhibit "D" , and copies of the lab results are <br /> attached hereto as Exhibits "E-1 through 7" . <br /> The PHS/EHD is the lead regulatory agency (LRA) , and will <br /> continue in that role throughout the investigation and clean up <br /> unless the case is referred to the Regional Water Quality <br /> Control Board. <br /> C. Work to Be Performed <br /> The chosen consultant will be required to provide all <br /> manpower, equipment, supplies, and licenses or permits required <br /> to perform the following work: <br /> TASK 1 PLANNING & PERMITS <br /> A. Conduct a site background investigation to obtain the <br /> information necessary to carry out the filed work and to <br /> prepare the site background section of the report described in <br /> Task 5 . <br /> B. Prepare a Health and safety (H&S) Plan according to the <br /> PHS/EHD UST Corrective Action Site Safety Guidelines . <br /> C. Prepare project workplan pursuant to the Guidelines . <br /> D. Submit the H&S Plan and workplan and schedule to the <br /> PHS/EHD for review. <br /> TASK 2 SOIL BORING & MONITORING WELL INSTALLATION & SAMPLING <br /> A. Drill the soil borings and install the monitoring wells <br /> that the consultant has determined are required for the PSA { <br /> -2- <br />
