r J • �� <br /> D. Bidding Requirements <br /> Prospective consultants must be agreeable to or meet the <br /> following specifications : <br /> 1 . The consultant must have at least three years <br /> experience conducting underground storage tank corrective <br /> action work in California. A statement of qualifications <br /> attesting to such experience must accompany any bid submitted. <br /> 2 . It is preferred, but not required, that the consultant <br /> have an office within 45 miles of the corrective action site. <br /> 3 . The consultant must employ qualified persons to perform <br /> the work described above, including licensed geologists and <br /> other technical professionals and possess all necessary <br /> licenses to operate within the State of California and the <br /> jurisdiction of San Joaquin County. A list of qualified persons <br /> who will perform the work must accompany any bid submitted. <br /> 4 . The consultant must carry out the described work in a <br /> timely, professional, safe and legal manner and maintain <br /> necessary records and other safeguards to ensure that reported <br /> sample results are accurate. All work must be performed in a <br /> manner acceptable to PHS/EHD. <br /> 5 . The consultant must provide at least three (3) <br /> references of previous clients for whom the consultant has <br /> performed similar work within the past two (2 ) years . <br /> 6 . The consultant must maintain general liability <br /> insurance in the amount of $500,000 . 00 and professional errors <br /> and omissions liability insurance in the amount of <br /> $1,000, 000 . 00 for the life of the project. Mr. tsirelas shall <br /> not be responsible for the cost of any associated premiums . <br /> Consultant shall be responsible for any third party claims <br /> related to the execution of the work described herein, with the <br /> exception of claims filed by any employees of Mr. Tsirelas . <br /> Proof of such insurance shall be furnished to Mr. Tsirelas <br /> prior to the start of work. <br /> 7 . Work shall be performed in a manner which will not <br /> require the cessation of business at the site unless absolutely <br /> necessary or dictated by the PHS/EHD. <br /> 8 . Consultant shall provide a copy of consultant ' s <br /> standard services agreement and schedule of charges with any <br /> bid submitted. The schedule of charges shall include ( 1) hourly <br /> rates for personnel classifications employed by consultant, (2 ) <br /> -4- <br />
