v TC <br /> www.atcassociates.com <br /> V <br /> has been received, ATC will prepare and submit the appropriate well destruction permits, <br /> encroachment permit, off-site access agreement for wells located on the adjoining property, <br /> Master File Record form along with the required fees. <br /> Additionally, a site specific health and safety plan (HASP) will be prepared to address well <br /> abandonment activities. The HASP will be prepared in accordance with Title 8, Section 5192 of <br /> '— the California Code of Regulations. _. <br /> ATC will mark the site for USA North to assist markouts by utility companies in the work vicinity. <br /> V <br /> Wells to be abandoned are shown on Figure 2, Site Plan. <br /> 3.0 Well Abandonment Activities <br /> Drilling activities will be conducted by a drilling contractor licensed to abandon wells in the State <br /> 2 <br /> of California. Based on historical laboratory analytical results (Table 2), ATC proposes to <br /> abandon wells MW-3, MW-4, MW-6, MW-7, MW-8 and MW-11 by pressure grouting the wells <br /> from the bottom up. The top 3 feet of well casing will be removed and approximately 6 inches of <br /> material will be removed from below the new top of casing. The grout sealant will be allowed to <br /> over flow the new top of casing and fill the excavated area to a minimum depth of 6 inches <br /> above the top of casing, creating a mushroom cap shaped seal at least one foot in depth. The <br /> remaining wells are proposed to be destroyed by over-drilling the well to remove the screen, <br /> �- filter pack and bentonite seal. The empty boreholes will then be grouted from the bottom up to V 5' <br /> approximately six inches below the top of grade. Final surface coverage will be matched to �t ' <br /> surrounding conditions. <br /> 4.0 Drill Cuttings Description, Sampling and Storage <br /> During well destruction activities, investigation derived waste (IDW) will be collected and <br /> y temporarily stored at the site in 55-gallon drums with sealable lids. IDW will be properly <br /> characterized and disposed of at an appropriate facility.
