Page 2 of 2 <br /> Eligible for Closure (Case No. 1) March 30, 2015 <br /> 375 West Hazelton Avenue, <br /> Stockton, CA 95203 <br /> After review of the subject site's historical analytical data presented by your consulting firm, <br /> AGE, the EHD has determined the following wells may be destroyed by the pressure grout <br /> method or over-drill: MW-3, MW-5, MW-6, and MW-8. Due to historical presence of petroleum <br /> product and/or fuel additive contaminants or insufficient groundwater and soil data, the following <br /> wells must be over-drilled: MW-1, MW-2, MW-4, MW-7, VW-1 through VW-6, and AW-1 through <br /> AW-5. <br /> To proceed with well destruction permit issuance the following must be submitted to the EHD to <br /> the attention of Gabriel Gonzalez: <br /> • Well destruction work plan, including table(s) detailing well construction (e.g. total depth, <br /> borehole diameter, etc.) and planned destruction method, and a site plan depicting all <br /> well locations; <br /> • Two EHD Well and Boring Permit Application forms (one for each parcel); <br /> • Master File Record form; <br /> • Licensed Contractor's Declaration (LCD); and <br /> • A fee of $1,170. The EHD permit and inspection fees have changed. Additional <br /> inspection time spent beyond the initial pre-paid fee will be billed off the permits at the <br /> current hourly rate of$130. <br /> After EHD review of the work plan and issuance of the permit, the EHD requires inspections to <br /> be scheduled with Johnny Yoakum, REHS at least 5 workdays prior to implementing fieldwork. <br /> Mr. Yoakum may be contacted via email at JYoakum@sjcehd.com or(209)468-3147. <br /> In summary, the following items are to be submitted to EHD prior to well destruction activities: <br /> CSR, public notice draft, complete list of record owners of fee title (e.g. RTO form), well <br /> destruction work plan and well destruction permit items listed above. <br /> If you have any questions concerning this letter, please contact Gabriel Gonzalez at (209) 468- <br /> 3442, or by email at gagonzalez@sjcehd.com <br /> Sincerely, <br /> Gabriel Gonzalez, EHS Nuel Henderson, PG <br /> Environmental Health Specialist Engineering Geologist <br /> c: Timothy J.
