HAZELTON Ave. (CASE 2) Site Code: 1644
WORK PLAN: Additional Monitoring Well Installation Work Plan (Case 2)
DATED: November 15,2005
PREPARED BY: AGE
APPROVED: [X]
DISAPPROVED: [ 1
ADDENDUM NEEDED: [ ] Submit by:
ADEQUATE AND NECESSARY ELEMENTS OF THE WORK PLAN [H&S 25296.10 (c)(3)]:
1. To define the lateral and vertical extent of soil and groundwater contamination, two
additional shallow MWs and two deeper MWs will be installed and sampled.
2. Soil and groundwater samples will be collected and analyzed for:TPH-Gas, BTEX, and
gasoline additives including MTBE and 1,2 DCA.
3. After installation, MWs will be developed, surveyed and sampled quarterly.
4. A report of findings will be prepared and submitted to EHD and CVRWQCB for review and
comment.
COMMENTS AND CONDITIONS:
1. A deep MW is proposed in the former UST area where documented soil and groundwater
contamination resides. To reduce the chance of cross contaminating the deeper hydro
geologic unit, consider relocating the deeper MW downgradient immediately outside of the
heavily contaminated soil source area. The deeper MW screens must be completed/set
across permeable soil strata. If groundwater contamination is found in the deeper MWs,
additional deep MWs may be required.
2. A permit application and fees must be submitted to EHD and the MWs installed by January
31, 2006.
3. The MWs must be developed, surveyed, and sampled within 2 weeks of installation and a
report of finding submitted by March 31,2006.
4. All required information must be submitted electronically to the State Water Resources
Control Board Geo-Tracker data base.
DATED I a- l7/ O BY
Ce: AGE Harlin Knoll, Senior REHS (209)468-3442
