The <br /> Appendix A report discusses the site background in detail. An acceptable Fact Sheet was <br /> approved after several iterations between Ground Zero and RWQCB. <br /> On August 1, 2008, the Fact Sheet was sent to all parties (property owners and tenants, as <br /> applicable) within a 250-foot radius of the Site. There were no responses to the mailing within <br /> the 45 day public comment period. <br /> Ground Zero submitted the Report of Results of Public Participation and Workplan for Vapor <br /> Extraction Well Destruction dated September. 15, 2008. The workplan was approved by the <br /> RWQCB in a letter dated September 18, 2008. The following report discusses the destruction of <br /> VW la/lb and VW 2. <br /> I <br /> WELL DESTRUCTION <br /> VW 1a/lb was a nested pair of soil vapor extraction wells set at a total depth of 26 and 42 feet bgs, <br /> respectively. VW2 was a soil vapor extraction well set at a total depth of 44 feet bgs. These wells j <br /> were properly destroyed per San Joaquin County well destruction standards. There were no other <br /> wells associated with the Site. <br /> Vapor Extraction Well Destruction <br /> i <br /> Appropriate well destruction permits were obtained from the San Joaquin County Environmental <br /> Health Department (EHD). The RWQCB and EHD were notified prior to well destruction <br /> activities. The Site was cleared via Underground Service Alert 72 hours prior to drilling activities. <br /> i <br /> On October 8, 2008, a Ground Zero geologist under.the supervision of a California Professional <br /> Geologist supervised V&W Drilling, Inc. (C57 License #720904) during the destruction of soil <br /> vapor extraction wells VW la/lb and VW2. The work was overseen by Ms. Lori Duncan of the <br /> EHD. Well cluster VW la/lb was over-drilled to a depth of 42 feet below the ground surface <br /> (bgs) using 8-inch hollow stem augers kept centered over the deeper well by a guide rod. The <br /> boring was then over-drilled using 10-inch hollow stem augers. <br /> I <br /> VW2 was over-drilled to a depth of 44 feet bgs using a guide rod to keep the augers centered <br /> over the well casing. Both borings were backfilled to 3 feet below the surface with neat cement <br /> grout emplaced with a tremie pipe system.
