PRELIMINARY REPORT: <br /> 0 Legal description of property to include: <br /> Assessor's parcel Number <br /> Township, Range and Section <br /> Street Address <br /> Tank Owner, (Petroleum Owner) <br /> Contact Person <br /> U Map showing site location relatively to nearby landmarks (north <br /> toward top of page). <br /> Sketch of the site showing nearby streets and properties. <br /> U Layout of all tanks and piping systems properly labeled for <br /> content. <br /> 0 Results of the tank tests or initial site work performed. <br /> 0 Estimate of Contaminant(s) leaked into the environment. <br /> 0 Local topography, geology, nearby streams, depth to groundwater, <br /> shallow, unconfined and/or confined aquifer. <br /> 0 Proximity of private or municipal wells and irrigation wells. <br /> Location of nearby septic tanks, underground public utility <br /> lines. <br /> 0 Distance to adjacent buildings and water and sewer lines. <br /> 0 Proposed borehole, trenches, or monitor well location. <br /> Field investigation may begin only after the plan has been approved. <br /> All necessary permits for monitoring well construction and soil borings <br /> shall be secured prior to commencement of the construction.
