Waste Unit <br /> l <br /> RE: Monitoring Well Inspections <br /> The following guidelines apply to monitoring well inspections associated <br /> with site assessments and unauthorized release investigations: <br /> General <br /> 1 . The initial $35.00 permit fee paid at the time of application pays <br /> for one inspection or one hour of inspection whichever is less . <br /> Any additional inspection visits and/or time must be billed. Make <br /> sure the billing information is provided. <br /> 2 . If the grout cannot be completed before 4:30 PM the driller must stop <br /> work until the next working day or agree to pay overtime for time <br /> after 4 : 30 PM. Call in and notify supervisor you will be working <br /> overtime or receive authorization prior to leaving office to work <br /> overtime if responsible party willing to pay. <br /> 3. Grout inspections must be scheduled hrs in advance. <br /> M.W. Grout Inspections <br /> 1 . Identify well or boring # and location on site drawing. <br /> 2 . Document construciton details: <br /> a. total depth of boring <br /> b. screen length <br /> C . water table depth <br /> d. depth to top of sand/gravel pack <br />' <br /> C. depth to top of bentonite pellets <br /> f . depth of interval to be grouted <br /> g. any contamination noted such as odor or visual evidence <br /> 3 . Approved grout mixtures are cement/bentonite slurry or cement <br /> sand slurry described in attachment . Neat cement is also allowed <br /> at minimum of 7 sack mix . Any variation from these mixes must have <br /> been approved by another regulatory agency such as RWQCB , DUBS-TSCD <br /> and be identified in the approved work plan. <br /> 4 . Grout may be placed by free fall from surface if annular space above <br /> bentonite pellets is : <br /> a. less than 10 ft . <br /> or <br /> b. less than 15 ft . and top of bentonite is greater than 10 ft <br /> above water table . <br /> 5. All other grouts must be placed by tremie method or by using hollow <br /> stem auger as described on attached sheet. Any other method must <br /> iy have been approved by another regulatory agency and be identified <br /> on the approved work plan. <br /> 6. A concrete mixture with aggregate may be used for last 1j to 2 ft of <br /> grout if setting a Christy box or vault . <br />
