1 1 <br /> C) <br /> i, <br /> i� 28 <br /> i� <br /> i <br /> SITE SAFETY AND HEALTH PLAN I� <br /> I: <br /> .i <br /> The site history, project objectives, and site maD are included in the work plan. <br /> .i <br /> Key Personnel <br /> Mr. Larry Phillips is the Project Manager (PNI). Ms. Pam Bare, Certified Industrial <br /> Hygienist (CIH), is the Company Health and Safety Officer (HSO). Ms. Jeanne ;Wallberg <br /> is the Project Geologist and will also function as the Site Safety Officer (SSO). The <br /> project field staff have completed 40 hours of comprehensive health and safetyi training <br /> which meets the requirements of Title 29 Code of Federal Regulations Part 1910.120 (29 <br /> CFR 1910.120). The SSO has the authority to monitor and correct health and safety <br /> problems as noticed on site. <br /> i� <br /> The PM is responsible for generating, organi ing, and compiling the site safety and <br /> health plan (SSHP) which describes all planned fi ld activities and potential hazards that <br /> may be encountered at the site. The PM is also responsible for assuring that i adequate <br /> training and safety briefing(s) for the project are provided to the project team. The PM <br /> has provided a copy of this SSHP to each member f the project field team. ! <br /> i� <br /> The HSO is responsible for developing and coordinating the BC health and safety <br /> program. For specific projects, she is responsible for reviewing and approving l the draft <br /> SSHP for accuracy and incorporating new informat on or guidelines which aid the PM and <br /> SSO in further definition and control of the poten 'al health and safety hazards associated <br /> with the project. <br /> The Project Geologist is responsible for ensuring that all data acquisition is performed <br /> in accordance with the work plan and SSHP, and that deviations from the plan are based <br /> upon field conditions encountered and are well documented in the field notes. The Project <br /> Geologist's health and safety responsibilities includ : <br /> 1. Following the SSHP. <br /> 2. Reporting to the PM any unsafe conditio s or practices. <br /> .i <br /> 3. Reporting to the PM all facts pertaining to incidents which result in 1l injury or <br /> exposure to toxic materials. <br /> 4. Reporting to the PM equipment malfunctions or deficiencies. <br /> The SSO has on-site responsibility for ensuring that all team members,!i including <br /> subcontractors, comply with the SSHP. It is he SSO's responsibility to inform the <br /> p Y <br /> subcontractors and other field personnel of chemical and physical hazards !las he/she <br /> becomes aware of thein.
