1 Jan 05 <br /> Q-3- <br /> The <br /> The design was validated in a report submitted to the Board by Nolte Associates <br /> (Attachment B). This document, Additional Information Report For Report of Waste <br /> Discharge - November 2003, is also on file with the Board. <br /> Schenone contracted with D.A Parrish to build the system as described in the two <br /> documents above. Schenone, who was assisted by the project architect, Lesovsky- <br /> Donaldson, drew the physical layout based on the schematic design. The layout <br /> (Attachment C) was reviewed and approved by San Joaquin County. The settling tank <br /> was upgraded from 1200 gallons to 2000 gallons at this time, as it had already been <br /> included in Parrish's proposal. <br /> A building permit was issued to Parrish (Attachment D). San Joaquin County EHD Staff <br /> made numerous trips to the site to inspect the installation and witness various <br /> performance tests. The system received final inspection by EHD in February of 2004. <br /> Additional fees were charged (Attachment E) to Parrish for the large amount of time <br /> spent reviewing and inspecting the construction and testing of the installed system. <br /> As built drawings were approved by (Attachment F) Carl Buress at D.A Parrish. He <br /> personally estimated, quoted, installed, and commissioned the entire system. <br /> Photographic plates (Attachment G) show the flowmeter, control panel, and dispersal <br /> area. <br /> I hope this is sufficient to meet Provision F.1.a - Report Certifying New Wastewater <br /> System has been constructed as Described in WDR R5-2004-0034.
